胎儿心脏超声用于产前诊断胎儿先天性心脏病的临床价值分析

摘　　要：目的分析胎儿心脏超声用于产前诊断胎儿先天性心脏病的临床价值。方法选取2020年1月至2021年1月抚州市立医院妇产科收治的150例孕妇作为研究对象,所有孕妇均进行超声心动图检查。以分娩和引产尸检结果为金标准,分析其检出率、胎儿心脏畸形类型、心脏超声心动图的灵敏度、特异度和准确度。结果所有孕妇经分娩或引产尸检后,10例胎儿被诊断为心脏异常,检出率为6.67%。心脏超声心动图发现先天性心脏病9例,漏诊1例,灵敏度为90.00%,特异度为100.00%,诊断符合率为99.33%;引产及分娩检查结果显示10例心脏异常胎儿中有4例为室间隔缺损,2例为法洛四联症,大动脉转位、单心室、心内膜垫缺损、及主、肺动脉狭窄各1例。心脏超声心动图除漏诊1例主、肺动脉狭窄外,其他与金标准结果一致。结论心脏超声心动图应用于产前诊断胎儿先天性心脏病的临床诊断中效果显著,检出率高,可为临床确诊提供充分依据。Objective To analyze the clinical value of fetal cardiac ultrasound in prenatal diagnosis of congenital heart disease.Methods 150 pregnant women admitted to the department of Obstetrics and Gynecology of Fuzhou Municipal Hospital from January 2020 to January 2021 were selected as the study subjects,all pregnant women underwent echocardiography.The detection rate,the type of fetal heart malformation,the sensitivity,specificity and accuracy of echocardiography were analyzed with the results of delivery and induction of labor as the gold standard.Results After delivery or induced labor,10 cases were diagnosed with cardiac abnormality,the detection rate was 6.67%.9 cases of congenital heart disease were found by echocardiography,and 1 case of missed diagnosis,the sensitivity was 90.00%,specificity was 100.00%,and diagnosis coincidence rate was 99.33%;the results of labor induction and delivery showed that among the 10 cases of abnormal heart,4 cases were ventricular septal defect,2 cases were tetralogy of Fallot,and 1 case was major artery transposition,single ventricle,endocardial pad defect,and the stenosis of main and pulmonary artery,respectively.The results of echocardiography were consistent with the gold standard except for 1 case of missed diagnonsis of main and pulmonary artery stenosis.Conclusion Echocardiography is effective in prenatal diagnosis of congenital heart disease,and the detection rate is high,which can provide sufficient basis for clinical diagnosis.
