摘 要:目的观察并分析儿童青少年近视等效球镜度(SE)与黄斑中心凹脉络膜厚度(SFCT)、视网膜神经节细胞-内丛状层(GCIPL)厚度、眼轴长度(AL)的相关性研究。方法回顾性病例研究。选取2021年1月至2022年7月就诊于北京市普仁医院眼科的8~14岁的儿童青少年,睫状肌麻痹验光测量屈光度,按照SE分为低度近视组(>-3 D且<-0.5 D)、中度近视组(>-6 D且≤-3 D)、高度近视组(≥-12 D且≤-6 D)和对照组(>-0.5 D且<0.5 D),并测量SFCT、GCIPL六个方位厚度(上方、鼻上、鼻下、下方、颞下、颞上)、GCIPL平均厚度、最小厚度和AL,评估其与近视屈光度数的关系。GCIPL厚度与AL采用Spearman相关系数分析,SFCT与AL采用广义估计方程分析。结果共纳入97例(142只眼),男性47例,女性50例,年龄(11.2±1.8)岁。GCIPL厚度与AL的Spearman相关系数及GCIPL厚度与AL的Spearman相关系数分析结果显示:GCIPL厚度各个指标与AL均呈现出显著负相关(P<0.001),GCIPL上方,下方,颞下方及平均厚度与年龄显著负相关,其余指标和年龄均无显著相关。广义估计方程结果显示:AL的回归系数值为-24.846(Z=-4.522,P<0.001),意味着AL对SFCT有显著的负向影响关系。结论儿童青少年近视SE越深,AL越长,SFCT越薄,GCIPL越薄,并且GCIPL下方与SE程度相关性最显著。Objective To observe and analyze the correlation between the spherical equivalent(SE)in children and adolescents and the thickness of the foveal choroid layer(SFCT),the thickness of the retinal ganglion cell inner plexus layer(GCIPL),and the axial length(AL).Methods A retrospective study was conducted on children and adolescents aged 8 to 14 who visited the ophthalmology department of Puren Hospital in Beijing from January 2021 to July 2022.Atropine dilated pupil optometry was used to measure refractive power,and they were divided into the low myopia group(>-3 D and<-0.5 D),moderate myopia group(>-6 D and≤-3 D),high myopia group(≥-12 D and≤-6 D),and control group(>-0.5 D and less than 0.5 D)according to equivalent spherical(SE)power.SFCT and GCIPL thickness in six directions(upper,upper nasal,lower nasal,lower temporal,and upper temporal),GCIPL average thickness was measured.Minimum thickness,AL,evaluate its relationship with myopic diopter.GCIPL thickness and AL were analyzed using Spearman correlation coefficients,while SFCT and AL were analyzed using generalized estimation equations.Results A total of 97 people(142 eyes)were included,including 47 males and 50 females,aged 11.2±1.8 years old.The analysis of the Spearman correlation coefficient between GCIPL thickness and AL,as well as the Spearman correlation coefficient between GCIPL thickness and AL,showed that all indicators of GCIPL thickness showed a significant negative correlation with AL(P<0.001).The upper,lower,lower temporal,and average thickness of GCIPL were significantly negatively correlated with age,while the other indicators were not significantly correlated with age.The results of the generalized estimation equation show that the regression coefficient of AL was-24.846(Z=-4.522,P<0.001),indicating a significant negative impact of AL on SFCT.Conclusions The deeper the SE,the longer the AL,the thinner the SFCT,and the thinner the GCIPL in children and adolescents with myopia,and the most significant correlation between the degree of SE is
