摘 要:目的 探讨多模态超声检查特征对乳腺癌Ki-67、趋化素样因子MARVEL跨膜结构域包含6(CMTM6)表达的预测价值。方法 选取接受手术治疗及病理证实的145例乳腺癌患者,术前进行常规超声检查、剪切波弹性成像(SWE)和超声造影(CEUS)检查,免疫组织化学法检测Ki-67、CMTM6的表达。多因素logistic回归分析多模态超声检查特征对Ki-67、CMTM6表达的预测价值。结果 乳腺癌组织Ki-67、CMTM6表达阳性率分别为90.34%(131/145)、88.97%(129/145),均高于癌旁正常乳腺组织9.66%(14/145)、11.03%(16/145),差异有统计学意义(χ^(2)=198.7、199.1,P均<0.001)。常规超声检查肿块形态、肿块边界和有无微钙化与乳腺癌Ki-67、CMTM6表达差异均有统计学意义(P均<0.05)。SWE征象中硬环征、黑洞征与乳腺癌Ki-67、CMTM6表达差异均有统计学意义(P均<0.05)。CEUS特征以早增强、晚消退灌注与乳腺癌Ki-67、CMTM6表达差异均有统计学意义(P均<0.05)。logistic回归分析显示,肿块形态不规则、边界不清楚、微钙化、硬环征、黑洞征及早增强、晚消退灌注是乳腺癌Ki-67、CMTM6阳性表达的独立预测因素。结论 多模态超声检查特征可用于预测乳腺癌Ki-67、CMTM6的表达,从而为乳腺癌的诊治提供依据。Objective To investigate the predictive value of multimodal ultrasound features for Ki-67 and chemokine like fac-tor MARVEL transmembrane domain containing 6(CMTM6)expression in breast cancer.Methods 145 breast cancer patients who received surgical treatment were examined by conventional ultrasound,shear wave elastography(SWE)and contrast-enhanced ultrasound(CEUS)before surgery.The expression of Ki-67 and CMTM6 was detected by immunohistochemistry.Multi-variate logistic regression analysis was used to analyze the predictive value of multimodal ultrasound features on Ki-67 and CMTM6 expression.Results The positive rates of Ki-67 and CMTM6 in breast cancer tissues were 90.34%(131/145)and 88.97%(129/145),respectively,which were higher than those in normal breast tissues adjacent to cancer 9.66%(14/145)and 11.03%(16/145)(χ^(2)=198.7,199.1,P<0.001).The tumor shape,tumor boundary and microcalcification in conventional ultra-sound were related to the expression of Ki-67 and CMTM6 in breast cancer(all P<0.05).The hard ring sign and black hole sign in SWE were related to the expression of Ki-67 and CMTM6 in breast cancer(P<0.05).The early enhancement and late regres-sion perfusion of CEUS characteristics were related to the expression of Ki-67 and CMTM6 in breast cancer(P<0.05).Logistic re-gression analysis showed that irregular tumor shape,unclear boundary,microcalcification,hard ring sign,black hole sign,early enhancement and late regression perfusion were independent predictors of Ki-67 and CMTM6 positive expression in breast cancer.Conclusion Multimodal ultrasound features can be used to predict the expression of Ki-67 and CMTM6 in breast can-cer,which is helpful for the diagnosis and treatment of breast cancer.
