摘 要:目的基于高斯图模型(Gaussian graphical model,GGM)构建头颈部鳞状细胞癌患者膳食模式网络,探讨该模式与头颈部鳞状细胞癌风险之间的关系。方法采用病例对照研究设计,应用简化版食物频率问卷(FFQ25)对121例经病理学确诊的原发头颈部鳞状细胞癌患者及242例同期未患头颈部鳞状细胞癌的人群进行问卷调查。基于GGM构建头颈部鳞状细胞癌患者膳食模式网络。依据中心性指标强度和膳食摄入量计算每个研究对象的膳食模式网络评分,采用多因素Logistic回归分析膳食模式网络评分与头颈鳞癌风险的关系。结果基于GGM得到1个主要的膳食模式网络:蔬菜河鲜类模式。校正混杂因素后发现,蔬菜河鲜类模式可降低头颈鳞癌患病风险(OR=0.121,95%CI=0.052~0.28,P<0.001)。结论基于GGM构建的蔬菜河鲜类模式可降低头颈部鳞状细胞癌风险。未来应进一步开展基于人群的前瞻性研究,以验证膳食摄入与头颈部鳞状细胞癌风险的关系。Objective To construct a dietary pattern network for patients with head and neck squamous cell carcinoma using Gaussian graphical models(GGM),and to explore its relationship with the risk of head and neck squamous cell carcinoma.Methods A case-control study design was employed,utilizing a simplified version of the food frequency questionnaire(FFQ25)to conduct a survey among 121 pathologically confirmed primary head and neck squamous cell carcinoma patients and 242 individuals without head and neck squamous cell carcinoma during the same period.The dietary pattern network scores for each study subject were calculated based on the intensity of centrality index and dietary intake.Multivariate Logistic regression was used to analyze the relationship between the dietary pattern network score and the risk of head and neck squamous cell carcinoma.Results Based on the GGM,one primary dietary pattern network was identified:the vegetable and river type model.After adjusting for confounding factors,it was found that the vegetable and river type pattern was associated with a reduced risk of head and neck squamous cell carcinoma(OR=0.121,95%CI=0.052~0.28,P<0.001).Conclusions The vegetable-river fish dietary pattern constructed using the GGM can reduce the risk of head and neck squamous cell carcinoma.Future prospective population-based studies should be conducted to validate the association between dietary intake and the risk of head and neck squamous cell carcinoma.
