玻璃酸钠联合卡波姆治疗TAO合并眼表损害临床疗效分析

摘　　要：目的:探讨卡波姆眼用凝胶联合玻璃酸钠滴眼液治疗甲状腺相关眼病(TAO)合并眼表损害的临床疗效。方法:选取2017年3月~2023年5月皖南医学院第二附属医院收治的34例(68眼)TAO合并眼表损害的患者作为观察对象,用随机数字法按先后顺序分为对照组、研究组各17例。对照组使用玻璃酸钠滴眼液,研究组在对照组基础上,继续联用卡波姆眼用凝胶治疗,比较两组临床疗效及治疗前后眼表损害相关指标的变化。结果:研究组患者治疗总有效率(97.05%)高于对照组(76.47%),P<0.05。干预治疗4周后,两组患者泪膜破裂时间(BUT)、SchirmerⅠ试验(SIT)分值均高于治疗前(P<0.05);研究组患者角膜荧光染色检查(FL)、眼表疾病评分指数(OSDI)分值均低于治疗前(P<0.05),对照组患者OSDI分值低于治疗前(P<0.05);且研究组患者干预治疗后BUT、FL、SIT、OSDI分值改善程度均优于对照组患者(P<0.05)。结论:卡波姆眼用凝胶联合玻璃酸钠滴眼液治疗TAO合并眼表损害的疗效显著,有利于加强治疗效果及保证泪膜稳定效果,改善不适症状,值得临床推广应用。Objective:To investigate the clinical efficacy of sodium hyaluronate eyedrops plus carbomer ophthalmic gel in treating thyroid-associated ophthalmopathy(TAO)with ocular surface damage.Methods:Thirty-four TAO patients(68 eyes)complicated with ocular surface damage admitted to our hospital were enrolled between March 2017 and May 2023,and randomly divided into control group(n=17)and study group(n=17)using the random number method.Patients in control group were treated with sodium hyaluronate eyedrops,and those in study group received additional carbomer gel.The two groups were compared regarding the clinical efficacy and changes in ocular surface damage indicators before and after treatment.Results:The overall effective rate of treatment in the study group was significantly higher than that in the control group(97.05%vs.76.47%,P<0.05).After 4 weeks of intervention,patients in both groups showed higher tear break-up time(BUT)and Schirmer I test(SIT)scores compared to before treatment(P<0.05).Patients in the study group showed lower cornea fluorescein staining(FL)and ocular surface disease index(OSDI)scores than before treatment(P<0.05),and those in the control group exhibited a decrease in OSDI scores(P<0.05).The improvement in BUT,FL,SIT,and OSDI scores after intervention was significantly better in the study group compared to the control group(P<0.05).Conclusion:Sodium hyaluronate eyedrops plus carbomer ophthalmic gel has significant efficacy in treating TAO with ocular surface damage.Their combined use can enhance the therapeutic effect and ensure the stability of tear film,thereby improving the subjective and objective discomfort symptoms of the observed subjects,which is worthy of clinical recommendation.
