基于共享决策家属参与式围术期护理对脑深部电刺激治疗帕金森病的影响

摘　　要：目的探讨基于共享决策家属参与式围术期护理对脑深部电刺激(deep brain stimulation,DBS)治疗帕金森(Parkinson’sdisease,PD)患者的影响方法DBS治疗的86例PD患者随机均分为2组,对照组43例,采用常规护理;观察组43例,在对照组基础上采用基于共享决策的家属参与式围术期护理。比较两组患者干预前后汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)、汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)、帕金森病生活质量问卷(Parkinson’sDisease Questionnaire-39,PDQ-39)评分以及患者家属干预前后的中文版共享决策-9问卷(9-item shared decision making questionnaire,SDM-Q-9)评分。结果干预后,两组患者HAMA、HAMD、PDQ-39评分均下降,观察组评分均低于对照组(P<0.05);干预后,两组患者家属SDM-Q-9评分均上升,观察组干预后SDM-Q-9评分高于对照组(P<0.05)。结论基于共享决策的家属参与式围术期护理可有效缓解DBS治疗PD患者负性情绪,对提高患者生活质量和家属共享决策程度有积极作用。Objective To investigate the effect of family participation perioperative nursing based on shared decision-making on patients with Parkinson's disease(PD)undergoing deep brain stimulation(DBS).Methods A total of 86 PD patients undergoing DBS were randomly divided into two groups,with 43 patients in each group.The control group received routine nursing care,while the observation group received family participation perioperative nursing based on shared decision-making in addition to routine care.The Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD),and Parkinson's Disease Questionnaire-39(PDQ-39)scores were compared between the two groups before and after intervention.Additionally,the Chinese version of the 9-item Shared Decision Making Questionnaire(SDM-Q-9)was administered to family members before and after intervention.Results After intervention,the HAMA,HAMD,and PDQ-39 scores decreased in both groups,with significantly lower scores observed in the observation group compared to the control group(P<0.05).Furthermore,the SDM-Q-9 scores of family members increased in both groups post-intervention,with a higher score in the observation group compared to the control group(P<0.05).Conclusions Family participation perioperative nursing based on shared decision-making can effectively alleviate negative emotions in PD patients under‐going DBS,positively impacting their quality of life and enhancing the degree of shared decision-making among family members.
