加速康复外科背景下结直肠癌D3根治术后乳糜漏护理对策应用的临床研究

摘　　要：目的探讨腹腔镜辅助结直肠癌D3根治术后并发乳糜漏的护理对策应用。方法本研究回顾性分析福建医科大学附属协和医院结直肠外科2020年5月至2022年5月腹腔镜辅助结直肠癌D3根治术后乳糜漏患者的临床资料。对患者采用禁食、全肠外营养支持、生长抑素治疗及加速康复外科理念下的护理对策。结果在2047例患者中,84例(4.1%)患者出现了乳糜漏。诊断时间平均为术后3.3 d。治愈时间平均5.8 d,治愈率达100%。结论加速康复外科的理念在术后乳糜漏的护理中具有关键作用。通过科学合理的饮食管理、引流护理、营养支持和心理护理,达到缩短患者的治愈时间、降低相关并发症发生率、减少医疗费用的目的。Objective This study aims to investigate the effectiveness of nursing strategies in managing chylous leak following laparoscopic radical D3 resection of colorectal cancer.Methods This study retrospectively analyzed the clinical data of patients with chylous leak after laparoscopic radical D3 resection of colorectal cancer in the Department of Colorectal Surgery,Fujian Medical University Union Hospital from May 2020 to May 2022.The patients were treated with fasting,total parenteral nutrition support,somatostatin treatment,and nursing strategies under the concept of enhanced recovery after surgery.Results Among 2047 patients,84(4.1%)developed chylous leak.The mean time to onset was 3.3 days after surgery.The mean time to resolution was 5.8 days,and the healing rate was 100%.Conclusions The implementation of accelerated rehabilitation surgery is crucial in the nursing care of patients with postoperative chylous leakage.By employing evidence-based approaches such as dietary management,drainage nursing,nutritional support,and psychological care,the objective is to expedite patient recovery,minimize the occurrence of associated complications and reduce healthcare expenses.
