探讨血清circNRIP1对宫颈癌的辅助诊断价值

摘　　要：目的探讨宫颈癌(CC)患者血清circRNA核受体相互作用蛋白1(circNRIP1)的表达水平和临床价值。方法收集2020年7月至2022年6月我院确诊的88例原发性CC患者、50例术后患者、40例复发转移CC患者、80例CIN患者、100例健康者的血清标本。采用实时荧光定量PCR的方法检测血清circNRIP1的表达水平,采用Mann-Whitney检验比较不同组间血清circNRIP1表达水平的差异,采用χ^(2)检验分析其与临床病理特征和CC传统辅助诊断指标的相关性,用ROC曲线及AUC评估circNRIP1、CEA、CA125以及SCCA的诊断效能。Kaplan-Meier生存曲线分析circNRIP1表达水平与CC患者预后的关系。结果原发性CC患者血清circNRIP1的相对表达量显著高于CIN患者和健康对照(P均<0.001),CIN患者高于健康对照者(P<0.001)。CC术后患者血清circNRIP1相对表达量明显低于术前患者(P<0.0001),复发和转移的CC患者血清circNRIP1相对表达量明显高于原发性CC患者和术后CC患者(P=0.0392;P<0.0001)。CC患者血清circNRIP1相对表达量与CEA、SCCA之间有相关性(r^(2)=0.145,P=0.0003;r^(2)=0.163,P=0.0001),且与患者的FIGO分期、淋巴结转移差异有统计学意义(P均<0.05)。CC患者和健康对照者比较,当circNRIP1、CEA、CA125以及SCCA四个指标联合检测时,敏感度高达98.9%,AUC为0.971。随访期间,CC患者血清circNRIP1的相对表达量术后呈明显下降趋势。Kaplan-Meier生存曲线显示血清circNRIP1高表达CC患者的总体生存率明显低于血清circNRIP1低表达患者(P=0.0466)。结论circNRIP1相对表达量可成为CC患者早期辅助诊断的标志物及疾病的恶性程度和进展的辅助观察指标,并对患者的预后判断有一定意义。Objective To investigate the expression level and clinical value of circRNA nuclear receptor interacting protein 1(circNRIP1)in serum of cervical cancer(CC)patients.Methods Serum samples were collected from 88 patients with primary CC,50 postoperative patients,40 patients with recurrent and metastatic CC,80 patients with CIN,and 100 healthy individuals diagnosed in our hospital from July 2020 to June 2022.Real time fluorescence quantitative PCR was used to detect the expression level of serum circNRIP1.Mann Whitney test was used to compare the differences in serum circNRIP1 expression levels between different groups.The correlation between it and clinical pathological features and traditional auxiliary diagnostic indicators of CC was analyzed,and the diagnostic efficacy of circNRIP1,CEA,CA125,and SCCA was evaluated using ROC curves and AUC.Kaplan-Meier survival curve analyzed the relationship between circNRIP1 expression level and prognosis in CC patients.Results The relative expression level of circNRIP1 in the serum of primary CC patients was significantly higher than that of CIN patients and healthy controls(P<0.001),and CIN patients were higher than healthy controls(P<0.001).The relative expression level of circNRIP1 in the serum of postoperative CC patients was significantly lower than that of preoperative CC patients(P<0.001),while the relative expression level of circNRIP1 in the serum of recurrent and metastatic CC patients was significantly higher than that of primary CC patients and postoperative CC patients(P=0.0392;P<0.001).There was a correlation between the relative expression level of circNRIP1 in the serum of CC patients and CEA and SCCA(r^(2)=0.145,P=0.0003;r^(2)=0.163,P=0.0001),and there was a statistically significant difference in FIGO staging and lymph node metastasis(P<0.05).Compared with healthy controls,CC patients showed a sensitivity of 98.9%and an AUC of 0.971 when combined with circNRIP1,CEA,CA125,and SCCA.During the follow-up period,the relative expression level of circNRIP1 in the se
