牙龈组织中巨噬细胞M1/M2极化相关因子的表达对重度慢性牙周炎患者的临床意义

摘　　要：目的探究重度慢性牙周炎患者牙龈组织中巨噬细胞M1/M2极化相关因子的表达及临床意义。方法收集96例重度慢性牙周炎患者的病损牙龈组织为研究组,并收集同一时间段80例健康牙龈组织(阻生牙冠方覆盖的牙龈组织)为对照组。比较两组患者牙龈组织中巨噬细胞M1/M2极化相关因子表达水平。治疗6个月后,根据重度慢性牙周炎患者治疗效果,分为疗效不佳组和疗效良好组。比较2组患者的临床资料,分析牙龈组织中巨噬细胞M1/M2极化相关因子对慢性牙周炎患者治疗效果的预测价值。结果研究组牙龈组织中诱导型一氧化氮合酶(iNOS)mRNA、信号传导及转录激活因子1(STAT1)mRNA、精氨酸酶Ⅰ(ArgⅠ)mRNA及信号传导及转录激活因子6(STAT6)mRNA表达量均高于对照组(P<0.05)。96例重度慢性牙周炎患者治疗6个月后疗效不佳发生率为12.50%。疗效不佳组附着丧失(AL)、简化牙石指数(CI-S)、iNOS mRNA、STAT1 mRNA、ArgⅠmRNA及STAT6 mRNA表达量均高于疗效良好组(P<0.05)。CI-S、iNOS mRNA、STAT1 mRNA及ArgⅠmRNA均为影响重度慢性牙周炎患者治疗效果的危险因素(P<0.05)。牙龈组织中iNOS mRNA、STAT1 mRNA及ArgⅠmRNA表达量对慢性牙周炎患者治疗效果进行预测的最佳截断点分别为1.16、1.21、0.87,曲线下面积(AUC)分别为0.772、0.776、0.812。结论牙龈组织中巨噬细胞极化相关因子iNOS mRNA、STAT1 mRNA及ArgⅠmRNA对重度慢性牙周炎预测价值高,临床中具有一定的参考价值。Objective To investigate the expression and clinical significance of macrophage M1/M2 polarization-related factors in the gingival tissue of patients with severe chronic periodontitis.Methods The damaged gingival tissues of 96 patients with severe chronic periodontitis were collected as the research group,and 80 healthy gingival tissues(gingival tissue covered by the crown of impacted teeth)were collected as the control group during the same time period.The expression levels of macrophage M1/M2 polarization-related factors in the gingival tissue of the two groups were compared.After 6 months of treatment,the treatment effects of patients with severe chronic periodontitis were evaluated and divided into the poor curative effect group and the good curative effect groups.The clinical data of patients were compared.The related factors affecting the treatment effect were analyzed by Logistic multivariate regression.A receiver operating characteristic curve(ROC)was used to analyze the predictive value of the macrophage M1/M2 polarization-related factors in the gingival tissue on the treatment effect of patients with chronic periodontitis.Results The expression levels of inducible nitric oxide synthase(iNOS)mRNA,signal transduction and activator of transcription 1(STAT1)mRNA,arginaseⅠ(ArgⅠ)mRNA and signal transduction and activator of transcription 6(STAT6)mRNA in the gingival tissues of the research group were higher than those in the control group(P<0.05).The incidence of poor efficacy in 96 cases of severe chronic periodontitis patients was 12.50%after 6 months of treatment.The expressions of attachment loss(AL),simplified tartar index(CI-S),iNOS mRNA,STAT1 mRNA,ArgⅠmRNA and STAT6 mRNA in the poor curative effect group were higher than those in the good curative effect group(P<0.05).Logistic multivariate analysis showed that CI-S,iNOS mRNA,STAT1 mRNA and ArgⅠmRNA were the risk factors affecting the treatment efficacy of patients with severe chronic periodontitis(P<0.05).ROC analysis showed that the optimal cu
