眼部移植物抗宿主病患者的角结膜特征

摘　　要：目的探讨眼部移植物抗宿主病患者角结膜特征。方法选取2022年10月至2023年8月期间,就诊安徽医科大学第一附属医院血液科,合并眼部移植物抗宿主病患者28例,分析其不同眼表不同区域角膜荧光素钠染色以及结膜充血分数。结果眼部移植物抗宿主病患者其鼻下象限角膜染色分数显著高于颞下象限[2.000(1.0,2.0)vs 1.000(1.0,1.0),Z=-2.132,P=0.033]及鼻上象限[2.000(1.0,2.0)vs 0.000(0.0,1.0),Z=-3.650,P<0.001],颞下象限角膜染色分数显著高于颞上象限[1.000(1.0,1.0)vs 1.000(0.0,1.0),Z=-1.995,P=0.045],颞侧结膜充血分数高于颞侧近角膜部[0.600(0.4,1.0)vs 0.250(0.2,0.6),Z=-3.967,P<0.001],鼻侧结膜充血分数高于鼻侧近角膜部[0.850(0.4,1.4)vs 0.400(0.2,0.7),Z=-2.827,P=0.005],鼻侧结膜充血分数高于颞侧[0.850(0.4,1.4)vs 0.600(0.4,1.0),Z=-0.699,P=0.485]。结论下方角膜染色分数较上方更显著,鼻下方角膜荧光素钠染色较颞下方更显著,球结膜充血程度较近角膜缘处结膜更显著,鼻侧结膜充血程度较颞侧更显著。Objective To explore the characteristics of corneal conjunctiva in patients with ocular graft-versus-hosts disease.Methods From October 2022 to August 2023,28 patients with ocular graft-versus-host disease who visited the Hematology Department of the First Affiliated Hospital of Anhui Medical University were selected to analyze their corneal fluorescein sodium staining and conjunctival injury scores in different ocular surfaces and regions.Results Patients with ocular graft-versus-host disease have significantly higher corneal staining scores under the nose than below the temporal region[2.000(1.0,2.0)vs 1.000(1.0,1.0),Z=-2.132,P=0.033]and above the nose[2.000(1.0,2.0)vs 0.000(0.0,1.0),Z=-3.650,P<0.001],meanwhile the corneal staining score under the temporal region is significantly higher than that above the temporal region[1.000(1.0,1.0)vs 1.000(0.0,1.0),Z=-1.995,P=0.045],The temporal conjunctival congestion score is higher than the temporal conjunctival congestion score near the corneal margin[0.600(0.4,1.0)vs 0.250(0.2,0.6),Z=-3.967,P<0.001],and the nasal conjunctival congestion score is higher than the nasal conjunctival congestion score near the corneal margin[0.850(0.4,1.4)vs 0.400(0.2,0.7),Z=-2.827,P=0.005],The nasal conjunctival congestion score is higher than the temporal side[0.850(0.4,1.4)vs 0.600(0.4,1.0),Z=-0.699,P=0.485].Conclusion The lower corneal staining score is more significant than the upper score,and the fluorescein sodium staining of the lower cornea is more significant than that of the temporal region.The degree of conjunctival congestion is more significant than that near the limbus,and the nasal conjunctival congestion score is more significant than the temporal side.
