摘 要:目的:探讨维生素D联合唑来膦酸对行内分泌治疗的绝经后乳腺癌患者骨代谢的影响。方法:抽取2020年1月至2022年1月商丘市第一人民医院收治的105例绝经后乳腺癌患者的临床资料进行有限元对照分析。根据治疗方法将105例患者分为观察组(54例,给予维生素D联合唑来膦酸治疗)和对照组(51例,给予唑来膦酸治疗)。比较两组临床疗效和治疗后骨密度、疼痛程度[视觉模拟评分法(VAS)评分]、骨代谢指标水平[骨钙素(OCN)、I型前胶原氨基端前肽(PINP)、抗酒石酸酸性磷酸酶5b(TRACP-5b)]、不良反应发生情况。结果:观察组治疗总有效率(96.30%,52/54)高于对照组(82.55%,42/51),P<0.05。观察组治疗后骨密度、血清OCN水平高于对照组,VAS评分、PINP水平、TRACP-5b水平低于对照组(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论:维生素D联合唑来膦酸可改善行内分泌治疗的绝经后乳腺癌患者的骨代谢状态,提高骨密度,有助于减轻疼痛,且安全性较好。Objective:To investigate the influence of vitamin D combined with zoledronic acid on bone metabolism in postmenopausal patients with endocrine therapy for breast cancer.Methods:The clinical data of 105 postmenopausal breast cancer patients admitted to First People’s Hospital of Shangqiu from January 2020 to January 2022 were selected for finite element control analysis.According to the treatment method,the 105 patients were divided into an observation group(54 cases,treated with vitamin D combined with zoledronic acid)and a control group(51 cases,treated with zoledronic acid).The clinical efficacy,postoperative bone mineral density,postoperative pain level assessed by visual analogue scale(VAS)score,postoperative levels of bone metabolism markers,including osteocalcin(OCN),type I procollagen amino-terminal propeptide(PINP)and tartrate-resistant acid phosphatase 5b(TRACP-5b),and incidence of adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the observation group(96.30%,52/54)was higher than that in the control group(82.55%,42/51),P<0.05.The bone mineral density and levels of serum OCN in the observation group were higher than those in the control group,while the VAS score,levels of PINP and TRACP-5b in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Vitamin D combined with zoledronic acid can improve the bone metabolism of postmenopausal patients with endocrine therapy for breast cancer,increase bone mineral density,and help reduce pain degree,with good safety.
