摘 要:目的探究肺泡灌洗联合纤维支气管镜吸痰治疗肺癌术后重症肺部感染的效果及对血气分析指标的影响。方法回顾性分析2020年1月至2023年12月郑州大学附属肿瘤医院行肺癌术后并发重症肺部感染患者60例的临床病理资料,根据治疗方法将其分为对照组和研究组,每组30例,对照组给予普通吸痰管吸痰,研究组给予肺泡灌洗联合纤维支气管镜吸痰。比较两组患者干预效果与血气指标水平。结果研究组干预总有效率(96.67%,29/30)高于对照组(80.00%,24/30),P<0.05。干预前两组动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、血氧饱和度(SaO_(2))水平比较差异未见统计学意义(P>0.05);干预后,两组PaO_(2)与SaO_(2)水平均升高,PaCO_(2)水平均下降,且研究组PaO_(2)与SaO_(2)水平高于对照组,PaCO_(2)水平低于对照组(P<0.05)。结论对肺癌术后并发重症肺部感染患者应用肺泡灌注联合纤维支气管镜吸痰,可提高干预效果,改善血气指标水平。ObjectiveTo investigate the efficacy of alveolar lavage combined with bronchoscopic sputum aspiration on severe pulmonary infection after lung cancer surgery and its influence on blood gas analysis indexes.MethodsThe clinicopathological data of 60 patients with severe pulmonary infection after lung cancer surgery in the Affiliated Cancer Hospital of Zhengzhou University from January 2020 to December 2023 were retrospectively analyzed.According to the treatment method,they were divided into a control group and a study group,with 30 cases in each group.The control group underwent sputum suction with ordinary sputum tube,and the study group was treated by alveolar lavage combined with bronchoscopic sputum aspiration.The intervention effect and levels of blood gas analysis indexes of the two groups were compared.ResultsThe total effective rate of intervention in the study group(96.67%,29/30)was higher than that in the control group(80.00%,24/30),P<0.05.There was no significant difference in arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),and arterial oxygen saturation(SaO_(2))between the two groups before intervention(P>0.05).After intervention,the levels of PaO_(2) and SaO_(2) in the two groups increased,while the level of PaCO_(2) in the two groups decreased(P<0.05);moreover,the levels of PaO_(2) and SaO_(2) in the study group were higher,while the level of PaCO_(2) was lower compared with the control group(P<0.05).ConclusionsAlveolar lavage combined with bronchoscopic sputum aspiration can improve the intervention effect on patients with severe pulmonary infection after lung cancer surgery,and improve the levels of blood gas indexes.
