经腹腔镜手术和开腹手术治疗低位直肠癌患者的效果比较

摘　　要：目的探讨经腹腔镜手术和开腹手术治疗低位直肠癌患者的应用价值。方法选取2020年2月至2022年2月赣南医科大学附属兴国医院收治的60例低位直肠癌患者为研究对象,按随机数字表法分作对照组及观察组,均30例。对照组行开腹治疗,观察组行腹腔镜手术,比较两组围手术期指标、肛门功能恢复情况、免疫水平及并发症发生率。结果观察组患者手术时间、肛门排气时间及住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);两组患者肛门功能恢复情况比较,差异无统计学意义(P>0.05);两组患者治疗前CD4^(+)、CD4^(+)/CD8^(+)、IgA及IgM比较,差异无统计学意义(P>0.05),观察组患者治疗后CD4^(+)、CD4^(+)/CD8^(+)、IgA及IgM高于对照组,差异有统计学意义(P<0.05);观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论低位直肠癌应用经腹腔镜手术可获确切临床效果,且免疫功能影响较小,并发症发生率低。Objective To explore the application value of laparoscopic and open surgery in the treatment of low rectal cancer patients.Methods A total of 60 patients with low rectal cancer admitted to Xingguo Hospital Affiliated to Gannan Medical University from February 2020 to February 2022 were selected as the research objects.According to the random number table method,they were divided into the control group and the observation group,with 30 cases in each group.Perioperative indexes,anal function recovery,immune level and complication rate were compared between the two groups.Results The operation time,anal exhaust time and hospital stay in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,the differences were statistically significant(P<0.05);There was no significant difference in the recovery of anal function between the two groups(P>0.05).Before treatment,there were no significant differences in CD4^(+),CD4^(+)/CD8^(+),IgA and IgM between the two groups(P>0.05),and after treatment,CD4^(+),CD4^(+)/CD8^(+),IgA and IgM in the observation group were higher than those in the control group,with statistical significances(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion The application of laparoscopic surgery for low rectal cancer can achieve exact clinical results,and the effect of immune function is small,and the incidence of complications is low.
