胃溃疡伴上消化道出血的胃镜下氩气刀结合金属钛夹治疗效果

摘　　要：目的探究胃溃疡伴上消化道出血治疗中胃镜下氩气刀结合金属钛夹的应用效果。方法非随机选取2020年3月—2023年10月东平县人民医院76例胃溃疡伴上消化道出血患者为研究对象,按不同的治疗方法分为两组。观察组38例患者接受胃镜下氩气刀结合金属钛夹治疗,对照组38例患者接受常规治疗。比较两组临床相关指标、胃功能指标、止血情况与不良反应发生情况。结果观察组患者完全止血、便血消失以及住院时间短于对照组,观察组胃泌素-17、胃蛋白酶原Ⅰ水平高于对照组,差异有统计学意义(P均<0.05)。观察组治疗后再次出血率低于对照组,有效止血率高于对照组,差异有统计学意义(P均<0.05)。观察组不良反应总发生率为2.63%(1/38),低于对照组的23.68%(9/38),差异有统计学意义(χ^(2)=7.370,P<0.05)。结论采用胃镜下氩气刀结合金属钛夹方案治疗胃溃疡伴上消化道出血患者,有效止血率更高,能改善胃功能,有助于降低治疗后不良反应的发生率。Objective To investigate the effect of gastroscopic argon knife combined with metal titanium clips in the treatment of gastric ulcer with upper gastrointestinal bleeding.Methods Seventy-six patients with gastric ulcer with upper gastrointestinal bleeding in Dongping County People's Hospital were non-randomly selected as study subjects from March 2020 to October 2023,and were divided into two groups according to different treatment methods.Thirty-eight patients in the observation group received gastroscopic argon knife combined with metal titanium clip treatment,and 38 patients in the control group received conventional treatment.The two groups were compared in terms of clinical related indexes,gastric function indexes,hemostasis and the occurrence of adverse reactions.Results The complete hemostasis,disappearance of blood in stool and hospitalization time of patients in the observation group were shorter than those in the control group,and the levels of gastrin-17 and pepsinogen I in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The rate of rebleeding after treatment in the observation group was lower than that in the control group,and the rate of effective hemostasis was higher than that in the control group,and the differences were statistically significant(both P<0.05).The total incidence rate of adverse reactions in the observation group was 2.63%(1/38),lower with 23.68%(9/38)in the control group,and the difference was statistically significant(χ^(2)=7.370,P<0.05).Conclusion The use of a gastroscopic argon knife combined with a metal titanium clip protocol for the treatment of patients with gastric ulcers with upper gastrointestinal bleeding has resulted in a higher rate of effective hemostasis,improves gastric function,and helps to reduce the incidence of post-treatment adverse reactions.
