摘　　要：目的:探讨帕金森病(PD)患者血清促食欲素A(orexin-A)、胱抑素C(CysC)水平与睡眠障碍的相关性。方法:选取2022年7月—2023年12月安徽医科大学附属宿州医院收治的原发性PD患者94例为PD组,依据PD睡眠量表(PDSS)评分,将PD组患者分为睡眠障碍组(n=50)和无睡眠障碍组(n=44);选取同时期的健康者为对照组(n=74)。比较各组血清orexin-A、CysC水平,并分析血清orexin-A、CysC水平与睡眠障碍的相关性。结果:PD组血清orexin-A水平低于健康对照组,血清CysC水平高于健康对照组,差异有统计学意义(P<0.05);睡眠障碍组血清orexin-A、CysC水平均高于无睡眠障碍组,差异有统计学意义(P<0.05)。PD患者血清orexin-A、CysC水平与PDSS评分呈弱负相关(r=−0.381、−0.280,P<0.05)。血清orexin-A、CysC水平及联合预测PD伴睡眠障碍的曲线下面积分别为0.663[95%CI(0.552,0.775)]、0.769[95%CI:(0.673,0.866)]、0.832[95%CI(0.749,0.916)]。结论:PD患者血清orexin-A水平较低,CysC水平较高,orexin-A、CysC水平与PD患者的睡眠质量呈负相关,血清orexin-A、Cysc水平及二者联合对于预测PD患者发生睡眠障碍具有一定的价值。Objective To explore the correlation between serum levels of orexin-A and cystatin C(CysC)and sleep disorders in patients with Parkinson's disease(PD).Methods Select 94 cases of primary PD patients admitted to Suzhou Hospital of Anhui Medical University from July 2022 to December 2023 as the PD group.According to the Parkinson's Disease Sleep Scale(PDSS),the PD patients were divided into a sleep disorder group(n=50)and a non sleep disorder group(n=44);Selected healthy individuals from the same period as the control group(n=74).Collect data on the research subjects,detect the levels of orexin-A and CysC in the serum of each group,compare the differences in serum orexin-A and CysC levels among the groups,and analyze the correlation between serum orexin-A and CysC levels and sleep disorders.Results The serum orexin-A levels in the PD group were lower than those in the healthy control group,and the serum CysC levels were higher than those in the healthy control group(P<0.05);The serum levels of orexin-A and CysC in the sleep disorder group were higher than those in the non sleep disorder group(P<0.05);The serum levels of orexin-A and CysC in PD patients were weakly negatively correlated with PDSS scores(r=−0.381,−0.280,P<0.05);The area under the curve of orexin-A,CysC,and the combined prediction of PD with sleep disorders were 0.663[95%CI(0.552,0.775)],0.769[95%CI:(0.673,0.866)],0.832[95%CI(0.749,0.916)],respectively,indicating that the combined detection has better performance.Conclusions The level of serum orexin-A in PD patients is low,and the level of CysC is high.The levels of orexin-A and CysC are negatively correlated with the sleep quality of PD patients.The levels of serum orexin-A,CysC and their combination have certain value in predicting sleep disorders in PD patients.
