腹腔镜辅助与开腹胃癌根治术的临床效果及对肠道屏障功能的影响对比

摘　　要：目的探究腹腔镜辅助与开腹胃癌根治术临床效果及对肠道屏障功能影响。方法选取2022年3月至2024年3月江南大学附属医院科室胃肠外科收治的50例胃癌患者,随机分为对照组(给予开腹胃癌根治术治疗)和观察组(给予腹腔镜辅助胃癌根治术治疗),各25例。对比两组临床效果、手术指标、术后并发症。结果相比对照组,观察组治疗总有效率更高,P<0.05。观察组术中出血量更少,且术后首次排气时间及进食时间更短,P<0.05。两组治疗前血二胺氧化酶及血内毒素水平对比,P>0.05;两组治疗后,观察组血二胺氧化酶水平高于对照组,血内毒素水平低于对照组,P<0.05。观察组术后并发症总发生率较对照组更低,P<0.05。结论与开腹胃癌根治术相比,采用腹腔镜辅助胃癌根治术治疗胃癌患者可提升整体疗效,改善肠道屏障功能,加快患者康复速度,减少术后并发症。Objective:To explore the clinical effect of laparoscopic-assisted and open radical gastrectomy for gastric cancer and its influence on intestinal barrier function.Methods:From March 2022 to March 202450 patients with gastric cancer were randomly divided into control group(given open radical gastrectomy)and observation group(given laparoscopic-assisted radical gastrectomy),with 25 patients in each group.The clinical effects,surgical indexes and postoperative complications of the two groups were compared.Results:Compared with the control group,the total effective rate in the observation group was higher,P<0.05.The amount of blood loss in the observation group was less than that in the control group,and the first exhaust time and eating time after operation were shorter than those in the control group,P<0.05.Before treatment,the levels of blood diamine oxidase and blood endotoxin were compared between the two groups,P>0.05;After treatment,the blood diamine oxidase level in the observation group was higher than that in the control group,and the blood endotoxin level was lower than that in the control group,P<0.05.The total incidence of postoperative complications in the observation group was lower than that in the control group,P<0.05.Conclusion:Compared with open radical gastrectomy,laparoscopic-assisted radical gastrectomy can improve the overall curative effect,improve the intestinal barrier function,speed up the recovery of patients and reduce postoperative complications.
