Giant trichilemmal carcinoma of the scalp:A case report

摘　　要：报告1例巨大外毛根鞘癌。头皮结节10年,增大伴溃疡3年。皮肤科检查:头顶部一直径约15 cm的肿块,形态不规则,表面见血管扩张,局部溃疡,可触及波动感。皮损组织病理示鳞状上皮呈乳头状增生伴角化过度及角化不全,肿瘤呈分叶状结构,小叶周围细胞呈栅栏状排列,与周围间质界限分明,可见外毛根鞘型角化、透明细胞、核分裂象及异形细胞。诊断:外毛根鞘癌。行手术切除治疗。术后6个月后创面愈合正常。A case of large trichilemmal carcinoma was reported.A nodule was noted on the scalp for 10 years and was enlarged with ulceration for 3 years.Dermatological examination revealed a tumor with a diameter of 15 cm,irregularly shaped,with dilated vessels on the surface,focally ulcerated with purulent discharge and crust,on the scalp.The tumor was palpable with cystic sensation.Histopathological examination of the lesion showed epidermal hyperplasia with hyperkeratosis and parakeratosis.There was a dermal tumor with lobular arrangement.The tumor cells were arranged in a fence-like pattern,with a clear boundary.Trichilemmal keratinization can be seen.The tumor cells were atypical,with mitoses and clear cells changes.A diagnosis of trichilemmal carcinoma was made.Surgical resection was performed.The wound healed well 6 months after surgery.
