血清IGF-1、IL-18与食管癌患者放疗敏感性的相关性

摘　　要：目的探讨分析血清胰岛素样生长因子1(IGF-1)、白介素-18(IL-18)与食管癌患者放疗敏感性的相关性。方法回顾分析,采集2021年1月至2022年12月于焦作市第二人民医院接受放疗的98例食管癌患者的资料。患者均接受放疗,放疗结束1周后评估放疗敏感性,根据放疗敏感性分组,记录患者人口学资料与临床资料,分析两组放疗前IGF-1、IL-18与放疗敏感性的相关性。结果98例患者中,放疗敏感65例(66.33%)。放疗敏感组与放疗不敏感组的肿瘤分期、肿瘤分化、转移情况差异有统计学意义(P<0.05)。放疗不敏感组放疗前的血清IGF-1、IL-18水平高于敏感组(P<0.05)。点二列相关性检验显示,食管癌放疗前血清IGF-1、IL-18水平与放疗敏感性呈正相关(r>0,P<0.05)。经限制性立方样条模型(RCS)分析,食管癌放疗前血清IGF-1、IL-18与放疗不敏感风险的关联呈非线性J型剂量反应关系,当血清IGF-1、IL-18分别>截断值时,患者不敏感风险随指标水平升高而增加。结论食管癌患者放疗前的血清IGF-1、IL-18与放疗敏感性存在正相关,即放疗前高水平的IGF-1、IL-18水平可能提示放疗不敏感高风险,应引起重视。Objective To explore and analyze the correlation between serum insulin like growth factor 1(IGF-1),interleukin-18(IL-18)and radiosensitivity in esophageal cancer patients.Methods Retrospective analysis was conducted to collect data from 98 esophageal cancer patients who underwent radiotherapy at the Second People’s Hospital of Jiaozuo from January 2021 to December 2022.All patients received radiotherapy,and radiosensitivity was evaluated one week after the end of radiotherapy.Patients were grouped according to radiosensitivity,and demographic and clinical data were recorded.The correlation between IGF-1,IL-18 and radiosensitivity before radiotherapy was mainly analyzed in both groups of patients.Results Among all 98 patients,65 cases(66.33%)were sensitive to radiotherapy.There were statistical differences in tumor staging,tumor differentiation,and metastasis between the radiation sensitive group and the radiation insensitive group(P<0.05).The expression of serum IGF-1 and IL-18 levels before radiotherapy in the insensitive group were higher than those in the sensitive group(P<0.05).Two column correlation test showed a positive correlation between the expression of serum IGF-1 and IL-18 levels before radiotherapy and radiosensitivity in esophageal cancer(r>0,P<0.05).According to the restricted cubic spline model(RCS)analysis,the correlation between serum IGF-1 and IL-18 before radiotherapy and the risk of radiation insensitivity in esophageal cancer showed a nonlinear J-type dose-response relationship.When serum IGF-1 and IL-18 were greater than the cut off value,the risk of patient insensitivity increased with the increase of indicator levels.Conclusion There is a positive correlation between serum IGF-1 and IL-18 levels before radiotherapy and radiosensitivity in esophageal cancer patients,indicating that high levels of IGF-1 and IL-18 levels before radiotherapy may indicate a high risk of radiotherapy insensitivity and should be taken seriously.
