低分子肝素联合苯扎贝特治疗高脂血症性急性胰腺炎的效果

摘　　要：目的观察低分子肝素联合苯扎贝特治疗高脂血症性急性胰腺炎(HLAP)的效果。方法选取2022年10月至2023年10月在新乡医学院第一附属医院接受治疗的80例HLAP患者为对象,采用随机数字表法分为两组。对照组接受禁食、营养支持、抗感染、抑制胰酶分泌等对症治疗,观察组在对症治疗基础上接受低分子肝素联合苯扎贝特治疗。比较两组临床效果。结果观察组肠鸣音恢复时间、腹胀和腹痛缓解时间、腹膜炎体征消失时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。两组治疗后甘油三酯、游离脂肪酸、血淀粉酶、尿淀粉酶、脂肪酶、载脂蛋白B及凝血功能指标均较治疗前降低,观察组治疗后均低于对照组,差异有统计学意义(P<0.05)。观察组多器官功能障碍综合征(MODS)发生率低于对照组,入住ICU率和28 d病死率与对照组比较,差异无统计学意义(P>0.05)。结论低分子肝素联合苯扎贝特治疗HLAP可调节血脂代谢,降低血尿淀粉酶,改善凝血功能,降低MODS的发生。Objective To observe the effect of low molecular weight heparin combined with benzabate in the treatment of hyperlipidemic acute pancreatitis(HLAP).Methods A total of 80 patients with HLAP treated in the First Affiliated Hospital of Xinxiang Medical College from October 2022 to October 2023 were selected and divided into two groups by random number table method.The control group received symptomatic treatment such as fasting,nutritional support,anti-infection,inhibition of pancreatic enzyme secretion.The observation group was given low molecular weight heparin combined with benzabate on the basis of symptomatic treatment.The clinical effect of the two groups was compared.Results The recovery time of intestinal sound,relief time of abdominal distension and abdominal pain,disappearance time of peritonitis signs and hospitalization time in the observation group were shorter than those in the control group,with statistical significance(P<0.05).After treatment,triglyceride,free fatty acid,blood amylase,urine amylase,lipase,apolipoprotein B and coagulation function indexes in 2 groups were lower than before treatment,and the observation group were lower than the control group,the difference was statistically significant(P<0.05).The incidence of multiple organ dysfunction syndrome(MODS)in the observation group was lower than that in the control group,and there was no significant difference in ICU admission rate and 28 days mortality between the observation group and the control group(P>0.05).Conclusion Low molecular weight heparin combined with benzabate in the treatment of HLAP can regulate lipid metabolism,reduce hematuric amylase,improve coagulation function and reduce the occurrence of MODS.
