基于“六腑以通为用”论治缺血性脑卒中

摘　　要：缺血性卒中是最常见的急性脑血管意外,属于中医学“卒中”范畴,是严重危害身心健康的重大慢性病。“六腑以通为用”理论源于《黄帝内经》,是中医学理论的重要组成部分。六腑以通为用,以降为顺,若六腑不通不降,则脾胃、肠腑、三焦气机受阻,气机不畅,浊邪内生,上逆犯脑导致中风。六腑闭塞不通,气机受阻,清浊升降失常,水谷精微流失于下,日久气血亏虚,外邪易犯,人体正气虚弱和虚邪之风两虚相得,亦可发为中风。现代研究发现,缺血性卒中与肠道菌群、神经通路、内分泌通路、免疫机制等密切相关。基于六腑以通为用理论提出缺血性卒中的治疗以通腑泻浊、开窍醒神为主,体现了中医脑肠共治、上病下治的治疗理念。Ischemic stroke is the most common acute cerebrovascular accident,which belongs to the category of“cuzhong”in traditional Chinese medicine.It is a major chronic disease that seriously harms physical and mental health.“The six fu-organs must keep their unobstructed”originates from the Huangdi Neijing and is an important part of the theory of traditional Chinese medicine.The six fu-organs must keep their unobstructed and descending.If the six fu-organs are blocked and not descending,the spleen and stomach,the large intestine and small intestine,and Qi activity of triple energizers are blocked.Blocked Qi activity causes endogenous turbid pathogenic factor,which causes stroke by attacking the brain upwardly.Obstructed six fu-organs and blocked Qi activity lead to abnormal ascending and descending of clearness and turbidity.Food nutrients are easily lost,Qi and blood will be deficient,and external pathogens are easy to invade the body.The combination of weak healthy Qi and deficient pathogens can cause stroke.Modern researches have found that ischemic stroke is closely related to intestinal flora,neural pathways,endocrine pathways and immune mechanisms.Based on the theory of“the six fu-organs must keep their unobstructed”,the treatment of ischemic stroke is mainly based on dredging the fu-organs to expel turbidity,and causing resuscitation and awakening consciousness,which reflects the treatment concepts of brain and intestine co-treatment and treating upper body diseases through the lower body.
