摘　　要：目的:构建稳定期慢性阻塞性肺疾病(COPD)病人肺康复锻炼依从性干预方案并评价其可行性。方法:以健康行为改变整合理论(ITHBC)为框架,通过文献研究和半结构化访谈拟定干预方案初稿,经德尔菲专家咨询法修订方案。采用目的抽样法在北京市某三级医院门诊选取稳定期COPD病人,随机分为试验组和对照组实施干预,对照组采用常规护理干预,试验组按照构建的干预方案指导病人实施肺康复并评价其可行性。结果:构建的干预方案包括准确评估、知信合“依”、自我成长和“依”路相伴4个主题、17条内容。干预12周,试验组呼吸和运动锻炼依从性高于对照组(P<0.05)。结论:基于ITHBC理论构建的稳定期COPD病人肺康复锻炼依从性干预方案具有较好的临床可行性和实用性,有助于提高病人的呼吸和运动锻炼依从性。Objective:To construct and evaluate the feasibility of pulmonary rehabilitation exercise compliance intervention scheme for patients with stable chronic obstructive pulmonary disease(COPD).Methods:Based on the Integrated Theory of Health Behavior Change(ITHBC),the first draft of the intervention plan was developed through literature research and semi⁃structured interviews,and the plan was revised by the Delphi expert consultation method.Objective Sampling method was used to select stable COPD patients in the outpatient department of a tertiary hospital in Beijing,and randomly divided into experimental group and control group to implement intervention and evaluate its feasibility.The control group carried out routine nursing intervention,and the experimental group guided the patients to carry out pulmonary rehabilitation according to the constructed intervention plan.Results:The constructed intervention plan included 4 themes,which were accurate assessment,knowledge,trust and"dependence",self⁃growth and"dependence",and 17 contents.After 12 weeks intervention,the compliance of breathing and exercise in the observation group was higher than those in the control group(P<0.05).Conclusion:The ITHBC theory based pulmonary rehabilitation exercise compliance intervention scheme for stable COPD patients has good clinical feasibility and practicability,which is helpful to improve patients'breathing and exercise compliance.
