摘　　要：体外生命支持(ECLS)技术的发展推动了现代医学的进步,为诸多疾病的救治提供了有效支撑,同时也引发了新的医学伦理学论题。ECLS的获益评价、资源分配和治疗终点等方面的伦理学论题逐步成为医患双方甚至全社会共同面临的困境。明晰并以患者利益为中心提出此类伦理学矛盾的最优解需要多方的协同促进与监督,由此维护医学的公平和对患者的尊重。本文针对ECLS应用相关的伦理学现状进行回顾总结,以期为ECLS的合理应用提供伦理学参考。The technological and scientific development of extracorporeal life support(ECLS)has promoted the progress of modern medicine,provided an effective platform for the treatment of critical diseases,and raised new medical ethics issues at the same time.Ethical issues such as treatment benefit,resource allocation,and treatment endpoints of ECLS have become a dilemma faced by the whole society,especially doctors and patients.Proposing a solution to such ethical issues based on the interests of patients requires the coordinated promotion and supervision of multiple parties so as to maintain fairness and respect for patients in medicine.This review summarizes the evidence of the current ethical issues with ECLS in order to provide a reference for clinicians optimizing the application of ECLS.
