失笑散联合胃复春治疗慢性萎缩性胃炎的疗效及对血清免疫受体和炎症因子水平的影响

摘　　要：目的观察失笑散联合胃复春治疗慢性萎缩性胃炎的疗效及对血清Toll样受体4(TLR4)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)水平的影响。方法选取2019年9月至2022年4月深圳市罗湖区人民医院收治的慢性萎缩性胃炎患者92例,根据不同治疗方式分为观察组(n=46)和对照组(n=46),对照组采用胃复春治疗,观察组在对照组的基础上联合失笑散治疗。比较两组治疗有效率、血清TLR4、IL-4、TNF-α水平、木村-竹本分型评分及不良反应发生率。结果治疗3个月后,观察组的治疗有效率显著高于对照组、中医证候评分均低于对照组(P<0.05);观察组血清TLR4、IL-4、TNF-α[(12.76±2.38)μg/mL、(32.62±6.18)ng/L、(1.16±0.34)pg/mL]水平均显著低于对照组[(23.16±5.36)μg/mL、(38.34±6.15)ng/L、(1.38±0.22)pg/mL](P<0.05);观察组木村-竹本分型评分均显著低于对照组(P<0.05);观察组的不良反应发生率低于对照组(P<0.05)。结论失笑散联合胃复春能够有效缓解机体氧化应激反应,缓解症状,避免不良反应的发生,提高治疗有效率。Objective To investigate the efficacy of Shixiao powder and Weifuchun in the treatment of chronic atrophic gastritis and its effect on serum Toll-like receptor 4(TLR4),interleukin-4(IL-4)and tumor necrosis factor-α(TNF-α)levels.Methods A total of 92 patients with chronic atrophic gastritis admitted to Shenzhen Luohu District People's Hospital from September 2019 to April 2022 were selected and divided into an observation group(n=46)and a control group(n=46)based on different treatment methods.The control group received treatment with Weifuchun,while the observation group received treatment with Shixiaosan in addition to the control group.The treatment efficacy,serum TLR4,IL-4,and TNF-αlevel,Kimura Takemoto classification score,and incidence of adverse reactions were compared between the two groups.Results After 3 months of treatment,the effective rate of treatment in the observation group was significantly higher than that in the control group,and the traditional Chinese medicine syndrome score was lower than that in the control group(P<0.05).The levels of the serum TLR4,IL-4,and TNF-αin the observation group[(12.76±2.38)μg/mL,(32.62±6.18)ng/L,and(1.16±0.34)pg/ml]were significantly lower than those in the control group[(23.16±5.36)]μg/mL,[(38.34±6.15)ng/L,(1.38±0.22)pg/ml](P<0.05).The Kimura Takemoto classification score in the observation group was significantly lower than that in the control group(P<0.05),and the incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion The combination of Shixiao power and Weifuchun can effectively alleviate the body's oxidative stress response,alleviate symptoms,avoid adverse reactions,and improve treatment efficiency.
