摘　　要：目的:探讨应用超声引导下经皮肝囊肿穿刺聚桂醇硬化治疗单纯性较大肝囊肿的临床价值。方法:以定点连续抽样的方法纳入金华市人民医院2019年6月至2022年3月肝胆胰外科就诊的单纯性较大肝囊肿患者240例,按照随机原则分为两组,观察组(120例)采用超声引导下经皮肝囊肿穿刺聚桂醇硬化术治疗,对照组(120例)采用腹腔镜肝囊肿开窗引流术治疗,对比两组手术指标、住院指标、临床疗效、并发症、生活质量。结果:与对照组相比,观察组患者手术时间较短、术中出血量较少(P<0.05);观察组患者住院时间短于对照组、住院花费低于对照组(P<0.05);观察组总有效率高于对照组(P<0.05);观察组不良反应发生率低于对照组(P<0.05);观察组简明健康生活质量量表(SF-36)总分高于对照组(P<0.05)。结论:在单纯性较大肝囊肿患者中采用超声引导下经皮肝囊肿穿刺聚桂醇硬化治疗能够取得良好的临床疗效,能够促进患者术后恢复,改善患者生活质量。Objective:Exploring the clinical value of ultrasound-guided percutaneous puncture and sclerotherapy with poly(cinnamyl alcohol)in the treatment of simple large liver cysts.Methods:A total of 240 patients with simple large hepatic cysts treated in the Department of hepatobiliary and pancreatic surgery of our hospital from June 2019 to March 2022 were included in the method of fixed-point continuous sampling,and were randomly divided into two groups.The observation group(120 cases)received ultrasound-guided percutaneous hepatic cyst puncture with lauromacrosclerotherapy,while the control group(120 cases)received laparoscopic hepatic cyst fenestration and drainage.The surgical indexes,hospitalization indexes,clinical efficacy,complications,and quality of life were compared between the two groups.Results:Compared with the control group,the operation time of the observation group was shorter and the intraoperative blood loss was lower(P<0.05).The hospitalization time of the observation group was shorter than the control group,and the hospitalization cost was lower than the control group(P<0.05).The total effective rate of the observation group was higher than that of the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).The total score of concise health quality of life scale(SF-36)in observation group was higher than that in control group(P<0.05).Conclusion:Ultrasound-guided percutaneous cholecystectomy with laurosinol can achieve good clinical effect in patients with simple large hepatic cysts,which can promote postoperative recovery and improve patients′quality of life.
