摘 要:目的探讨不同矫治器对成人正畸拔牙矫治患者牙根及牙槽骨的影响。方法选取濮阳市第三人民医院2018年1月至2023年1月收治的64例对称拔除第一前磨牙的成人正畸拔牙矫治患者为研究对象,按随机数字表法分成隐形组与固定组,每组32例。隐形组采用无托槽隐形矫治器,固定组采用传统固定矫治器,采用锥形束CT(CBCT)测量2组矫治前后不同位置牙根吸收及牙槽骨高度状况。结果隐形组矫治后左上中切牙、左上侧切牙、右上中切牙、右上侧切牙、左下中切牙、左下侧切牙、右下中切牙、右下侧切牙处牙根吸收均显著低于固定组矫治后(P<0.05);2组矫治后唇侧、舌侧、近中、远中处牙槽骨高度比较差异无统计学意义(P>0.05)。结论与传统固定矫治器相比,无托槽隐形矫治器应用于成人正畸拔牙矫治患者可有助于改善牙根吸收状况,对于牙槽骨影响接近。Objective To investigate the effect of different orthodontic devices on the roots and alveolar bone of adult orthodontic extraction patients.Methods 64 adult orthodontic extraction patients with symmetrical extraction of the first premolar admitted to the Third People’s Hospital of Puyang City from January 2018 to January 2023 were selected for the study,and they were divided into the invisible group and the fixed group according to the method of randomized numerical table,with 32 cases in each group.In the invisible group,bracketless invisible aligners were used,and in the fixed group,conventional fixed aligners were used.Cone beam CT(CBCT)was used to measure the root resorption and alveolar bone height in different positions before and after the treatment in the two groups.Results Root resorption at upper left central incisors,upper left lateral incisors,upper right central incisors,upper right lateral incisors,lower left central incisors,lower left lateral incisors,lower right central incisors,and lower right lateral incisors was significantly lower in the invisible group than that in the fixed group(P<0.05);the difference in the alveolar bone heights of labial,lingual,proximal-medial,and distal-medial teeth was not statistically significant between the 2 groups after orthodontic treatment(P>0.05).Conclusions Compared with conventional fixed aligners,bracketless invisible aligners for adult orthodontic extraction patients can help improve root resorption,and their impacts on the alveolar bone may be similar to conventional fixed aligners.
