摘 要:目的:评价基于术式绘制手术技术插图的教学方法在肾癌伴下腔静脉癌栓微创手术教学中发挥的作用。方法:选取2023年1月至2023年12月在解放军总医院第三医学中心泌尿外科医学部进行进修和规培的40名医师,将其随机分为实验组和对照组,每组20例,进行肾癌合并下腔静脉癌栓手术策略及技术讲解。实验组采用针对手术绘制的技术插图进行教学;对照组采用传统解剖图谱进行教学。比较两组学员的基线资料、教学后个人评价指标、教师对学员评价指标及教学前后客观测试分数。结果:两组学员基线资料比较差异无统计学意义(P>0.05)。经培训,实验组学员手术理解程度、疾病诊疗思路、主动学习意愿、与以往学习比较、个人评价总分均优于对照组,差异有统计学意义(均P<0.05)。教师对学员评价方面,实验组学习积极性、临床诊疗思维、治疗方法决策及评价总分均优于对照组,差异有统计学意义(均P<0.05)。授课前,两组学员测试分数比较差异无统计学意义(P=0.856);授课后,两组成绩均较授课前提升(均P<0.001),实验组授课后成绩优于对照组(P=0.007)。结论:基于术式绘制手术技术插图的教学效果优于传统解剖图谱教学,适合在泌尿外科手术技术教学及推广中应用。Objective:To evaluate the effectiveness of using surgical illustrations in teaching minimally invasive surgery for renal cancer with inferior vena cava(IVC)tumor thrombus.Methods:Selected 40 surgeons with less experience trained in the Senior Department of Urology,the Third Medical Center of the PLA General Hospital from January 2023 to December 2023,they were randomly divided into the experimental group and the control group,with 20 cases in each group,for learning surgical strategies and techniques of renal cancer with IVC tumor thrombus.The experimental group was taught using surgical illustrations,while the control group used traditional anatomical atlases.The baseline data,post-teaching self-assessment,teacher evaluations,and objective test scores before and after teaching were compared between the two groups.Results:There were no significant differences in baseline characteristics between the two groups(P>0.05).After training,self-assessed understanding of the surgery,disease management approach,willingness to engage in active learning,comparison with previous learning and total personal evaluation score in the experimental group were better than those in the control group,the differences were statistically significant(all P<0.05).In teacher evaluations of student engagement,the learning enthusiasm,clinical diagnosis and treatment decision-making,treatment strategy and total evaluation score in the experimental group were all better than those in the control group also,with statistically significant differences(all P<0.05).The pre-teaching test scores showed no significant difference between two groups(P=0.856).However,the post-teaching scores improved in both groups(both P<0.001),with the experimental group outperforming the control group(P=0.007).Conclusion:Teaching with procedure-specific technical illustrations is more effective than using traditional anatomical atlases,and is suitable for application and promotion in urological surgical technique education.
