摘　　要：目的:探讨慢性紧张型头痛(CTTH)患者颅颈伸肌变化与睡眠质量的关系。方法:选择10例CTTH患者(CTTH组)和10例健康受试者(健康受试组)。使用MRI检测颅颈伸肌的形态学变化,使用匹兹堡睡眠质量指数评估2组的睡眠质量。结果:2组在主观睡眠质量和日间功能障碍方面差异均有统计学意义(P=0.039,0.001)。2组主观睡眠质量、日间功能障碍与颅颈伸肌相对横截面积均无相关性(均P>0.05)。CTTH组的综合睡眠质量评分与头后小直肌的相对横截面积呈显著负相关(右侧为r=-0.705,P=0.023;左侧为r=-0.585,P=0.035)。结论:CTTH可能在主观睡眠质量和日间功能障碍这2个维度综合影响患者的睡眠质量,且CTTH患者头后小直肌的萎缩程度与睡眠质量相关,萎缩越重,睡眠质量越差。Objective:To explore the relationship between changes in cranio-cervical extensor muscles and sleep quality of patients with chronic tension-type headache(CTTH).Methods:A total of 10 CTTH patients(CTTH group)and 10 healthy individuals(control group)were collected.Morphological changes in the cranio-cervical extensor muscles were detected using MRI.Sleep quality was assessed using the Pittsburgh Sleep Quality Index(PSQI).Results:There were significant differences in subjective sleep quality score and daytime dysfunction score between the two groups(P=0.039,0.001).No correlations were found between subjective sleep quality score or daytime dysfunction score and the relative cross-sectional area of cranio-cervical extensor muscles(all P>0.05).The comprehensive sleep quality score in the CTTH group was negatively associated with the relative cross-sectional area of rectus capitis posterior minor(right,r=-0.705,P=0.023;left,r=-0.585,P=0.035).Conclusions:CTTH may effect the sleep quality in subjective sleep quality and daytime dysfunction,and the atrophy degree of rectus capitis posterior minor is related to sleep quality of CTTH patients,the more severe atrophy,the worse the sleep quality.
