IETA共识定义的超声征象在子宫内膜癌中的应用

摘　　要：目的:应用国际子宫内膜肿瘤分析(IETA)共识术语,描述子宫内膜癌的声像图特征与肿瘤分期、分级的关系,并通过其声像图特征分析超声漏诊的原因。方法:回顾性分析153例子宫内膜癌患者的超声图像,评估超声特征与病理结果之间的关系。结果:国际妇产科协会(FIGO)ⅠA期肿瘤多为高或等回声,ⅠB期及以上肿瘤多为混合性或低回声(P<0.001)。高回声在高中分化肿瘤中较常见,而非高回声在低分化肿瘤中较常见(P<0.001)。多支血管(灶性起源)多见于ⅠB期及以上肿瘤(P<0.001)和低分化肿瘤(P<0.001)。ⅠB期及以上(P<0.001)、低分化(P=0.018)肿瘤以较高血流评分为主。结论:子宫内膜癌的超声声像图表现与肿瘤的分期、分级密切相关。IETA共识定义的超声征象可应用于子宫内膜癌的术前评估。Objective:To describe the ultrasound features of endometrial cancer in relation to tumor stage and grade,using the International Endometrial Tumor Analysis(IETA)terminology.And to analyze the causes of ultrasound missed diagnosis by using the sonographic features.Methods:The ultrasound images of 153 patients with endometrial cancer were retrospectively analyzed to evaluate the relationship between ultrasound features and pathological findings.Results:Hyperechoic or isoechoic tumors were more common in FIGOⅠA cancer,whereas mixed echoic or hypoechoic tumors were more common in cancers of stageⅠB or greater(P<0.001).Hyperechogenicity was more common in highly or moderately differentiated tumors,whereas a non-hyperechoic appearance was more commonly found in poorly differentiated tumors(P<0.001).Multiple vessels were more common in tumors of stageⅠB or greater and poorly differentiated(both P<0.001).A high blood flow score was significantly more common in tumors of stageⅠB or greater(P<0.001)and poorly differentiated(P=0.018).Conclusions:The sonographic appearances of endometrial cancer are associated with tumor stage and grade.The ultrasound signs defined by IETA consensus can be applied to the preoperative evaluation of endometrial cancer.
