光活化消毒联合康复新液治疗中重度牙周炎的效果及对龈沟液炎性因子、生活质量的影响

摘　　要：目的探究光活化消毒联合康复新液治疗中重度牙周炎的效果及对龈沟液炎性因子、生活质量的影响。方法选取2021年1月至2023年6月郑州大学第一附属医院收治的104例中重度牙周炎患者作为研究对象,采用简单随机化法分为研究组和对照组各52例。两组患者均行龈下刮治和根面平整治疗,另给予对照组患者康复新液治疗,给予研究组患者光活化消毒联合康复新液治疗。比较两组患者的治疗效果,治疗前及治疗4周后的牙周指标(探诊深度、附着丧失度、菌斑及龈沟出血指数)、病原菌(牙龈卟啉单胞菌、福赛类杆菌、伴放线杆菌)、龈沟液炎性因子[白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、人类软骨糖蛋白-39(HCgp-39)、C-反应蛋白(CRP)]、生活质量[口腔健康影响程度量表(OHIP-14)评价]和咀嚼功能,同时比较两组患者治疗期间的不良反应情况。结果治疗4周后,研究组患者的临床总有效率为92.31%,高于对照组的76.92%,差异有统计学意义(P<0.05);治疗4周后,研究组患者的探诊深度、附着丧失度、菌斑及龈沟出血指数分别为(3.16±0.35)mm、(3.01±0.23)mm、(0.85±0.21)分、(0.72±0.22)分,明显低于对照组的(3.94±0.37)mm、(3.85±0.26)mm、(1.37±0.28)分、(1.08±0.32)分,差异均有统计学意义(P<0.05);治疗4周后,研究组患者的龈沟液IL-1β、TNF-α、IL-6、CRP、HCgp-39水平分别为(4.95±0.95)pg/mL、(3.74±0.51)ng/L、(5.37±0.61)ng/L、(6.42±1.76)mg/L、(15.53±2.49)ng/mL,明显低于对照组的(6.34±1.38)pg/mL、(4.48±0.69)ng/L、(6.45±0.94)ng/L、(8.83±1.91)mg/L、(19.79±2.88)ng/mL,差异均有统计学意义(P<0.05);治疗4周后,研究组患者的牙龈卟啉单胞菌、福赛类杆菌、伴放线杆菌检出率分别为7%、4%、1%,明显低于对照组的16%、12%、8%,差异均有统计学意义(P<0.05);治疗4周后,研究组患者的OHIP-14评分为(9.41±2.17)分,明显低于对照组的(13.54�Objective To explore the effect of photoactivated disinfection combined with Kangfuxin solution in the treatment of moderate and severe periodontitis and its influence on gingival crevicular fluid inflammatory factors and quality of life.Methods A total of 104 patients with moderate to severe periodontitis at the First Affiliated Hospital of Zhengzhou University from January 2021 to June 2023 were selected as study objects.They were divided into a study group and a control group using simple randomization,with 52 patients in each group.Both groups received subgingival scaling and root planing treatment.The patients in the control group were additionally treated with Kangfuxin solution,and the patients in the study group were treated with photoactivated disinfection combined with Kangfuxin solution.The treatment effects of the two groups,as well as the periodontal indicators(probing depth,attachment loss,plaque,and gingival bleeding index),pathogenic bacteria(Porphyromonas gingivalis,Fusobacterium fusiforme,Actinobacillus concomitatus),gingival crevicular fluid inflammatory factors(interleukin1β[IL1β],interleukin6[IL6],tumor necrosis factorα[TNFα],human cartilage glycoprotein39[HCgp39],Creactive protein[CRP]),quality of life(evaluated by the Oral Health Impact Profile14[OHIP14]),chewing function before and 4 weeks after treatment were compared between the two groups.The adverse reactions during treatment were also compared between the two groups.Results After 4 weeks of treatment,the total clinical effective rate in the study group was 92.31%,which was significantly higher than 76.92%in the control group(P<0.05).After 4 weeks of treatment,the probing depth,attachment loss,plaque bleeding index,and gingival sulcus bleeding index were(3.16±0.35)mm,(3.01±0.23)mm,(0.85±0.21)points,and(0.72±0.22)points,respectively,which were significantly lower than(3.94±0.37)mm,(3.85±0.26)mm,(1.37±0.28)points,and(1.08±0.32)points in the control group(P<0.05).After 4 weeks of treatment,the levels of IL1β,TNFα,IL6,CRP,and
