摘　　要：目的研究三阴性乳腺癌患者术前血清miR-218表达水平及术后生存情况,并分析两者间的关系。方法回顾性分析2021年1月至2022年12月期间焦作市人民医院收治的88例三阴性乳腺癌患者(观察组)的临床资料和术后生存情况,另选取60例经体检合格的健康女性作为对照组。采集三阴性乳腺癌患者术前血清和健康体检者血清并检测其miR-218表达水平;比较不同血清miR-218表达水平三阴性乳腺癌患者的临床病理特征;术后对观察组患者进行1年的随访,采用Kaplan-Meier生存曲线分析不同血清miR-218水平三阴性乳腺癌患者术后的生存情况;采用多因素Cox模型分析三阴性乳腺癌患者术后预后不良的影响因素。结果观察组患者的血清miR-218水平为1.98±0.69,明显低于对照组的3.24±0.93,差异有统计学意义(P<0.05);三阴性乳腺癌患者的年龄、月经状态、分化程度、淋巴结转移、组织学分级等病理特征不会影响患者血清miR-218水平(P>0.05),其血清miR-218水平仅与患者的肿瘤直径和TNM分期有关(P<0.05);三阴性乳腺癌患者术前血清miR-218高水平组1年累计生存率为95.65%(44/46),明显高于血清miR-218低水平组的78.57%(33/42),差异有统计学意义(P<0.05);多因素Cox回归分析结果显示,三阴性乳腺癌患者术前血清miR-218水平、淋巴结转移、TNM分期均是其术后预后的独立危险因素(P<0.05)。结论采用术前血清miR-218表达水平预测三阴性乳腺癌患者的生存情况具有很好的效能,且操作过程更加简便,具有临床应用价值。Objective To study the preoperative serum miR218 expression level and postoperative survival status in patients with triplenegative breast cancer(TNBC),and to analyze the relationship between the two.Methods The clinical data and postoperative survival status of 88 patients with TNBC(observation group)in Jiaozuo People’s Hospital from January 2021 to December 2022 were retrospectively analyzed.Another 60 healthy women undergoing physical examination were included in the control group.The serum of TNBC patients before surgery and serum of healthy subjects with physical examination were collected,and the expression level of miR218 was detected.The clinicopathological features were compared among TNBC patients with different serum miR218 expression levels.Patients in the observation group were followed up for 1 year after surgery.KaplanMeier survival curve was used to analyze the postoperative survival status of TNBC patients with different serum miR218 levels.Multivariate Cox model was adopted to analyze the influencing factors of postoperative poor prognosis in patients with TNBC.Results The level of serum miR218 in the observation group was 1.98±0.69,significantly lower than 3.24±0.93 in the control group(P<0.05).The age,menstrual status,differentiation degree,lymph node metastasis,histological grading,and other pathological features of TNBC patients did not affect the serum miR218 level(P>0.05),and serum miR218 level was only related to the tumor diameter and TNM staging of patients(P<0.05).The 1year cumulative survival rate was 95.65%(44/46)in preoperative high serum miR218 group,significantly higher than 78.57%(33/42)in low serum miR218 group(P<0.05).According to multivariate Cox regression analysis,preoperative serum miR218 level,lymph node metastasis,and TNM staging were independent risk factors for postoperative prognosis in TNBC patients(P<0.05).Conclusion Preoperative serum miR218 expression level has a good efficiency on predicting the survival status of patients with triplenegative breast cancer,and
