摘 要:目的:探寻增强MRI多模态定量参数及扩散加权成像(Diffusion weighted imaging,DWI)定量参数与原发性肝癌(Primary liver cancer,PLC)癌细胞恶性行为的相关性。方法:回顾性收集2019年1月—2023年10月我院收治的93例PLC患者的临床资料作为PLC组,另外收集同期50例肝脏良性疾病患者的临床资料作为对照1组,健康志愿者50例作为对照2组,比较3组及不同病理特征PLC患者间增强MRI多模态定量参数与DWI定量参数(T1、T2、灌注分数(f)、真实扩散系数(D)、灌注扩散系数(D*)、表观弥散系数(ADC))的差异。结果:PLC组患者的T1、f、D*值高于对照1组与对照2组,T2、D、ADC值低于对照1组与对照2组(P<0.05)。PLC组分期为Ⅲ~Ⅳ期、肿瘤直径≥5 cm、肝功能Child-Pugh分级为B级及C级患者的T1、f、D*值较高,T2、D、ADC值较低(P<0.05)。与癌旁组织比较,PLC组肝癌组织的HIF-1α、APE1、COL4A1、PI3K及Akt的mRNA表达量较高,FOXP2、p53的m RNA表达量较低(P<0.05)。Pearson相关性分析显示,T1、f、D*值与肝癌组织的HIF-1α、APE1、COL4A1、PI3K、Akt的mRNA表达量呈正相关,与FOXP2、p53的mRNA表达量呈负相关(P<0.05);T2、D、ADC值与肝癌组织的HIF-1α、APE1、COL4A1、PI3K、Akt的mRNA表达量呈负相关,与FOXP2、p53的mRNA表达量呈正相关(P<0.05)。结论:与肝脏良性疾病患者及健康人群比较,PLC患者增强MRI多模态定量参数及DWI定量参数发生显著变化,且相关定量参数均与PLC癌细胞恶性行为密切相关。Objective:To investigate the correlation between contrast-enhanced MRI multimodal quantitative parameters and diffusion weighted imaging(DWI)quantitative parameters and malignant behaviors of primary liver cancer(PLC)cells.Methods:The clinical data of 93 cases of PLC admitted to the hospital from January 2019 to October 2023 were retrospectively collected as the PLC group.Meanwhile,50 cases of benign liver diseases were selected as the control group 1,and 50 healthy volunteers were selected as the control group 2.The quantitative parameters(T1,T2,perfusion fraction(f),true diffusion coefficient(D),perfusion diffusion coefficient(D*)and apparent diffusion coefficient(ADC))of contrast-enhanced MRI and DWI were compared among the three groups and PLC patients with different pathological features.Results:T1,f and D*values of the PLC group were higher than those of the control group 1 and the control group 2.T2,D and ADC values were lower than those of the control group 1 and the control group 2(P<0.05).In the PLC group,T1,f and D*values of patients in stagesⅢ~Ⅳ,patients with tumor diameter≥5 cm and patients with Child-Pugh grade B and C liver function were higher.Their T2,D and ADC values were lower(P<0.05).In the PLC group,HIF-1α,APE1,COL4A1,PI3K,and Akt mRNA expression levels in cancer tissues were higher than those in adjacent tissues.FOXP2 and p53 mRNA expression levels were lower than those in adjacent tissues(P<0.05).Pearson correlation analysis found that T1,f and D*values were positively correlated with HIF-1α,APE1,COL4A1,PI3K and Akt mRNA,but negatively correlated with FOXP2 and p53 mRNA in liver cancer tissues(P<0.05).T2,D and ADC values were negatively correlated with HIF-1α,APE1,COL4A1,PI3K and Akt mRNA,but positively correlated with FOXP2 and p53 mRNA in liver cancer tissues(P<0.05).Conclusion:Compared with patients with be-nign liver diseases and healthy individuals,quantitative parameters of contrast-enhanced MRI and DWI show significant changes in patients with PLC.In addition,relevant quanti
