基于GBD数据库的全球20岁以下人群早产儿视网膜病变致中度视力损伤的负担分析

摘　　要：目的分析1990—2019年全球0~19岁儿童青少年早产儿视网膜病变(Retinopathy of Prematurity,ROP)致中度视力损伤(moderate vision loss,MVI)疾病负担的变化趋势,了解疾病负担现状,为ROP致盲的防控提供参考。方法数据来源于全球疾病负担研究数据库(GBD 2019),按照不同性别、年龄分析ROP相关MVI的患病率和伤残损失寿命年(YLD),评价疾病负担的变化情况。结果1990—2019年全球0~19岁儿童青少年ROP相关MVI患病人数从70.6万人增加到95.1万人,增加35%,患病率由29.8/10万增加至35/10万(年度平均变化百分比,AAPC=0.59;95%CI:0.58 to 0.61)。YLD标化率由0.9/10万人年增加至1.1/10万人年(AAPC=0.6;95%CI:0.58 to 0.61);在患病率和YLD标化率方面,女性和男性均呈上升趋势,且女性上升趋势大于男性(AAPC=0.62 vs 0.56)。结论全球ROP所致MVI的患病和负担仍呈上升趋势,其防控工作仍面临巨大挑战,应加强筛查和提高治疗水平,尤其是5岁以下新生儿的防控工作,降低儿童青少年ROP相关MVI的疾病负担。Objective To analyze the changing trend of the disease burden of moderate vision loss(MVI)caused by retinopathy of prematurity(ROP)in children and adolescents aged 0-19 years worldwide from 1990 to 2019,understand the current situation of the disease burden,and provide reference for the prevention and control of blindness caused by retinopathy of prematurity.Methods The data is sourced from the Global Burden of Disease(GBD 2019),and the incidence of moderate visual impairment caused by retinopathy in premature infants and Years of Life with Disability(YLD)are analyzed by gender and age to evaluate the changes in disease burden.Results From 1990 to 2019,the global prevalence of ROP related MVI in children and adolescents aged 0-19 increased from 706000 to 951000,an increase of 35%,and the prevalence rate increased from 29.8/100000 to 35/100000(AAPC=0.59;95%CI:0.58 to 0.61).The standardized rate of YLD increased from 0.9/100000 person years to 1.1/100000 person years(AAPC=0.6;95%CI:0.58 to 0.61).In terms of prevalence and standardized rate of YLD,both women and men showed an upward trend,and the upward trend in women was greater than that in men(AAPC=0.62 vs 0.56).Conclusion The global prevention and control of ROP blindness still faces enormous challenges,and the disease incidence and burden are still on the rise.It is necessary to strengthen screening and improve treatment levels,especially for premature infants under 5-year-old,to reduce the disease burden of ROP related MVI in children and adolescents.
