超声胃镜检查结合CA199检测在消化道早癌中的临床运用

摘　　要：目的研究超声胃镜检查结合CA199检测在消化道早癌中的临床运用。方法选取我院2020年1月—2022年12月收治的50例疑似消化道早癌患者作为研究对象,所有患者均接受超声胃镜检查,并以病理检查作为金标准,分析阳性率,同时检测患者血清癌抗原199(CA199),绘制ROC曲线分析超声胃镜检查结合CA199检测在消化道早癌的诊断价值。结果超声胃镜检查阳性40例,阳性率为88.88%,阴性5例;与阴性患者相比,阳性患者血清CA199水平显著更高,差异具有统计学意义(P<0.05);通过绘制ROC曲线分析得出:通过绘制ROC曲线分析得出:超声胃镜诊断消化道早癌灵敏度为88.90%,特异度为40.00%;血清CA199诊断消化道早癌灵敏度为86.70%,特异度为58.60%;两者联合诊断灵敏度92.30%和特异度80.00%均显著高于各项单一检测(P<0.05)。结论超声胃镜检查结合CA199检测在消化道早癌中的运用价值较高,可有效提高诊断价值。Objective To study the clinical application of ultrasonic gastroscopy combined with CA199 detection in early gastrointestinal cancer.Methods Fifty patients with suspected early gastrointestinal cancer admitted to our hospital from January 2020 to December 2022 were selected,and all patients underwent ultrasonic gastroscopy.Taking pathological examination as the gold standard,the positive detection rate of ultrasonic gastroscopy was analyzed.Serum cancer antigen 199(CA199)was detected in all patients,and receiver operating characteristic(ROC)curve was plotted to evaluate the diagnostic value of ultrasonic gastroscopy combined with CA199 detection in early gastrointestinal cancer.Results Ultrasonic gastroscopy showed that 40 cases were positive for early gastrointestinal cancer,with a positive rate of 88.88%,and 5 cases were negative.Serum CA199 level of positive patients was notably higher than that of negative patients(P<0.05).ROC curve analysis showed that the sensitivity and specificity of ultrasound gastroscopy in diagnosing early gastrointestinal cancer were 88.9%and 40.0%,respectively;The sensitivity and specificity of serum CA199 in diagnosing early gastrointestinal cancer were 86.7%and 58.6%,respectively;The combined diagnostic sensitivity of 92.30%and specificity of 80.00%were significantly higher than those of individual tests(P<0.05).Conclusion Ultrasound gastroscopy combined with CA199 detection has high value in the diagnosis of early gastrointestinal cancer,which can effectively improve the diagnostic value.
