新论肺热而颊赤的理论与实践

摘　　要：《内经素问·刺热篇》提出“肺热病者,右颊先赤”的重要学术观点,并未对治疗有深入论述。至宋钱乙《小儿药证直决》应用泻白散治疗肺经伏火后,针对肺热而颊赤理论历代医家多有发挥,在临床上有着重要的地位。但,临床实践中肺热而颊赤的肺热表现只是一种现象,其核心病机本质还存在“虚”的一面。因此,导师在川派外科钟以泽教授自创三皮汤系列治疗理论思想基础上,提出了肺热而颊赤,颊赤分虚实,颊赤有肺热,肺热存真假,辨证在求本,治疗必重虚的重要学术思想,并灵活加减茯苓白术汤用于该证,无论虚实之肺热,随证加减辨证均可使用,皆能获得较好疗效。The important academic viewpoint of“lung fever patients,right cheek first red”was put forward in“Internal Classic Question.”There was no in-depth discussion on treatment.After applying Xeibai powder to the treatment of lung meridian and fire,the theory of lung heat and cheek red has been played by many doctors in the past dynasties,which has an important position in clinical practice.However,in clinical practice,the manifestations of lung heat and cheek red lung heat are only a phenomenon,and there is still a“virtual”side of its core pathogenesis.Therefore,on the basis of the treatment theory of Sanpitang series created by Professor Zhong Yize of Sichuan School of Surgery,the instructor put forward the important academic thought that lung heat and cheek red,cheek red dividing deficiency and fullness,cheek red lung heat,lung heat remaining true and false,syndrome differentiation in the root,treatment must be heavy deficiency,and flexible addition and reduction of Poria Baizhu decoction for this syndrome,whether lung heat with deficiency and fullness,can be used with syndrome differentiation,and can obtain better curative effect.
