基于肠道干细胞稳态探讨痛泻要方治疗腹泻型肠易激综合征的机理

摘  要:肠道干细胞(intestinal stem cells, ISC)维持着肠道各种细胞的更新与平衡,影响着ISC的稳态。ISC稳态的维持则与肠道微生物、肠道屏障及肠道免疫系统密切相关。腹泻型肠易激综合征(IBS with predominant constipation, IBS-D)患者普遍存在肠道内分泌细胞异常、肠道菌群紊乱、肠粘膜屏障受损及肠道动力异常的情况,ISC稳态失衡可能是其发病的核心机制。根据中医理论脾“运化水谷精微”与“肝主疏泄”的功能和ISC稳态机理高度相似,研究发现痛泻要方可以从多个方面恢复失衡的ISC稳态。通过将中医“木旺克土”理论与ISC稳态相联系,提出痛泻要方治疗IBS-D的机制可能与恢复ISC稳态有关,为未来治疗IBS-D药物开发提供切入点。Intestinal stem cells(ISC)maintain the renewal and homeostasis of various cells in the intestine and influence ISC homeostasis,which is closely related to intestinal microorganisms,intestinal barrier and intestinal immune system.Patients with diarrhea-type irritable bowel syndrome(IBS-D)commonly have abnormalities of intestinal endocrine cells,intestinal flora disorders,impaired intestinal mucosal barriers,and abnormal intestinal motility.The imbalance of ISC homeostasis may be the core mechanism of it's pathogenesis.According to the theory of Chinese medicine,the functions of the spleen in“transporting water,grain and essence”and the liver in“draining and excretion”with the mechanism of ISC homeostasis are highly similar.It is found that the Tong-Xie-Yao-Fang can restore the imbalance of ISC homeostasis in many aspects.By linking the theory of Chinese medicine,“wood is strong and overcomes earth”with ISC homeostasis,it is suggested that the mechanism of Tong-Xie-Yao-Fang in treating IBS-D may be related to the restoration of ISC homeostasis,which may provide an entry point for the future development of drugs for treating IBS-D.
