生物羊膜覆盖联合自体角膜缘干细胞移植在翼状胬肉切除术患者中的应用效果

摘　　要：目的:观察生物羊膜覆盖联合自体角膜缘干细胞移植在翼状胬肉切除术患者中的应用效果。方法:选取2021年1月至2024年2月该院收治的76例翼状胬肉患者进行前瞻性研究,按照随机信封法将其分为研究组与对照组各38例。对照组应用自体角膜缘干细胞移植联合翼状胬肉切除术治疗,研究组在对照组基础上联合生物羊膜覆盖治疗,比较两组痊愈率、复发率、临床相关指标(角膜上皮修复时间、拆线时间)水平,手术前后视力水平、散光指标[角膜散光度、表面不对称指数(SAI)]水平、泪液炎性因子[白细胞介素-6(IL-6)、白细胞介素-2(IL-2)]水平,以及术后3个月并发症发生率。结果:研究组痊愈率为94.74%(36/38),高于对照组的78.95%(30/38),差异有统计学意义(P<0.05);两组复发率比较,差异无统计学意义(P>0.05);研究组角膜上皮修复时间、拆线时间均短于对照组,差异有统计学意义(P<0.05);术后3个月,研究组视力水平高于对照组,角膜散光度、SAI水平均低于对照组,差异有统计学意义(P<0.05);术后7 d,研究组泪液IL-6、IL-2水平均低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:生物羊膜覆盖联合自体角膜缘干细胞移植应用于翼状胬肉切除术患者可提高痊愈率和视力水平,改善临床相关指标水平,降低散光指标和泪液炎性因子水平,效果优于单纯自体角膜缘干细胞移植联合翼状胬肉切除术治疗。Objective:To observe application effects of biological amniotic membrane covering combined with autologous limbal stem cell transplantation in patients with pterygium excision.Methods:A prospective study was conducted on 76 patients with pterygium admitted to this hospital from January 2021 to February 2024.According to the random envelope method,they were divided into study group and control group,38 cases in each group.The control group was treated with autologous limbal stem cell transplantation combined with pterygium excision,while the study group was combined with biological amniotic membrane covering on the basis of those of the control group.The cure rate,the recurrence rate,the clinical related indexes(corneal epithelial repair time,suture removal time),the visual acuity before and after the surgery,the astigmatism index[corneal astigmatism,surface asymmetry index(SAI)]levels,the inflammatory factors in tears[interleukin-6(IL-6),interleukin-2(IL-2)]levels,and the incidence of complications at 3 months after the surgery were compared between the two groups.Results:The cure rate of the study group was 94.74%(36/38),which was higher than 78.95%(30/38)of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the recurrence rate between the two groups(P>0.05).The corneal epithelial repair time and the suture removal time in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).3 months after the surgery,the visual acuity of the study group was higher than that of the control group,the corneal astigmatism and SAI were lower than those of the control group,and the differences were statistically significant(P<0.05).7 days after the surgery,the levels of IL-6 and IL-2 in tears of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the
