咀嚼槟榔与胃食管反流病内镜下食管黏膜损伤程度分级及组织病理的关系分析

摘　　要：目的探讨咀嚼槟榔与胃食管反流病(GERD)内镜下食管粘膜损伤程度分级及组织病理的关系。方法选取2020年8月至2023年8月收治的196例GERD患者作为研究对象,所有患者均进行内镜检查,根据患者咀嚼槟榔情况分为轻度组、中度组和重度组,比较3组内镜下食管黏膜损伤程度分级和组织病理情况,并分析咀嚼槟榔与GERD内镜下食管黏膜损伤程度分级及组织病理的关系。结果轻度组食管黏膜损伤程度优于中度组和重度组(Z=38.85,P<0.05);轻度组病理组织学结果优于中度组和重度组(Z=31.99,P<0.05);Spearman相关性检验显示,咀嚼槟榔与GERD内镜下食管黏膜损伤程度分级和组织病理学结果均呈正相关(r=0.346;r=0.281,P<0.05)。结论咀嚼槟榔与GERD内镜下食管黏膜损伤程度分级和组织病理相关,可为临床GERD的诊断提供参考。Objective To explore the relationship between chewing areca nut and grading of endoscopic esophageal mucosal injury,histopathology in gastroesophageal reflux disease(GERD).Methods A total of 196 patients with GERD were enrolled as the research objects between August 2020 and August 2023.All patients underwent endoscopy.According to chewing degree of areca nut,patients were divided into mild group,moderate group and severe group.The grading of endoscopic esophageal mucosal injury and histopathology in the three groups were compared.The relationship between chewing areca nut and grading of endoscopic esophageal mucosal injury,and histopathology in GERD was analyzed.Results The esophageal mucosal injury in mild group was milder than that in moderate group and severe group(Z=38.85,P<0.05),and histopathology results were better than those in moderate group and severe group(Z=31.99,P<0.05).Spearman correlation test showed that chewing areca nut was positively correlated with the grading of endoscopic esophageal mucosal injury and histopathology results in GERD(r=0.346;r=0.281,P<0.05).Conclusion Chewing areca nut is related to the grading of endo-scopic esophageal mucosal injury and histopathology in GERD,which can provide a reference for clinical diagnosis of GERD.
