陕西省39家医疗机构宫颈细胞学液基制片质量与诊断水平现状分析

摘　　要：目的:了解陕西省各级医院病理科宫颈细胞学液基制片质量及诊断水平现况,为加强宫颈癌筛查工作规范性提供依据。方法:依托陕西省国际医学交流促进会,开展宫颈细胞学液基制片及诊断室间质量评价活动,同时对陕西省各级医院病理科宫颈细胞液基制片质量及诊断水平现况进行调查,分析并比较不同等级医院宫颈细胞液基制片优良率及诊断正确率。结果:陕西省内共39所各级医疗机构的宫颈细胞学液基制片质量及诊断水平数据纳入统计分析,根据《细胞学室间质量评价标准》评分,其中900分以上(优秀)占18家,占比46%,800分~900分(良好)13家,占比33%,600分~800分之间(合格)7家,占比17.9%,1家600分以下,占比2.5%。优良率79%,合格率97%。二甲医院优良率60%,三甲医院优良率84.6%。结论:陕西省内三级医院病理科宫颈细胞液基制片质量及诊断水平均较为规范,二级医院宫颈细胞液基制片优良率和诊断准确率有待提高,需相关部门对宫颈细胞液基制片质量及诊断水平进行规范化培训。Objective:To investigate the current status of cervical cytology liquid-based preparation and diagnosis in the pathology departments of hospitals at all levels in Shaanxi Province,and to provide a basis for strengthening the standardization of cervical cancer screening.Methods:Relying on Shaanxi International Medical Exchange and Promotion Association,external quality assessment(EQA)of cervical cytology liquid-based preparation and diagnosis was carried out.At the same time,the current status of cervical cytology liquid-based preparation and diagnosis in pathology departments of hospitals at all levels in Shaanxi Province was investigated.To analyze and compare the excellent and good rate of cervical cytology liquid-based preparation and the accuracy rate of diagnosis in hospital at all levels.Results:The quality data of cervical cytology liquid based preparation and diagnosis of 39 medical institutions at all levels in Shaanxi Province were included in the statistical analysis.According to the"Cytology External Quality Assessment Criteria",there were 18 medical institutions with scores above 900(accounting for 46%),13 medical institutions with scores between 800 and 900(accounting for 33%),and 7 medical institutions with scores between 600 and 800(qualified),accounting for 17.9%,and 1 medical institution was below 600 points(accounting for 2.5%).The excellent and good rate was 79%,and the qualified rate was 97%.The excellent and good rate was 60%in the grade A secondary hospital and 84.6%in the grade A teriary hospital.Conclusion:The liquid-based cervical cytology preparation and diagnosis in the pathology departments of the tertiary hospitals in Shaanxi Province are relatively standardized.The excellent and good rate and diagnostic accuracy of the liquid-based cervical cytology preparation in the secondary hospitals need to be improved,and the relevant departments need to carry out standardized training of the liquid-based cervical cytology preparation and diagnosis.
