化痰定眩方联合盐酸倍他司汀片治疗老年眩晕的效果及对脑血流速度的影响

摘　　要：目的:探讨化痰定眩方联合盐酸倍他司汀片治疗老年眩晕的临床效果及对患者脑血流速度的影响。方法:采用随机数字表法将2022年4月—2024年4月阳江市中医医院脑病科收治的80例老年眩晕患者分为对照组与试验组,各40例。对照组予常规治疗(对症处理、口服盐酸倍他司汀片),试验组在对照组基础上予化痰定眩方治疗,疗程为1个月。比较两组疗效、安全性及治疗前后症候积分、脑血流速度、血液流变学。结果:试验组总有效率高于对照组,差异有统计学意义(P<0.05)。两组治疗前症候积分比较,差异无统计学意义(P>0.05);治疗后试验组症候积分低于对照组(P<0.05)。两组治疗前脑血流速度比较,差异均无统计学意义(P>0.05);治疗后试验组左右侧椎动脉与基底动脉血流速度均高于对照组(P<0.05)。两组治疗前血液流变学指标比较,差异均无统计学意义(P>0.05);治疗后试验组血浆黏度、红细胞压积与全血高切黏度均低于对照组(P<0.05);两组均未发生严重不良反应。结论:化痰定眩方联合盐酸倍他司汀片治疗老年眩晕效果肯定,能够显著改善患者症候积分、脑血流速度与血液流变学。Objective:To investigate the clinical effect of Huatan Dingxuan Formula combined with Betahistine Hydrochloride Tablets in the treatment of senile vertigo and its influence on cerebral blood flow velocity.Method:A total of 80 senile vertigo patients admitted to Department of Brain Disease,Yangjiang Hospital of Traditional Chinese Medicine from April 2022 to April 2024 were divided into control group and experimental group by random number table method,with 40 cases in each group.The control group was given conventional treatment(symptomatic treatment and oral Betahistine Hydrochloride Tablets),and the experimental group was given Huatan Dingxuan Formula on the basis of the control group,the course of treatment was 1 month.The efficacy,safety,symptoms integrals,cerebral blood flow velocity and hemorheology before and after treatment were compared between the two group.Result:The total effective rate of experimental group was higher than that of control group,the difference was statistically significant(P<0.05).There was no significant difference in the symptoms integrals before treatment between the two groups(P>0.05);after treatment,the syndromes integrals of experimental group was lower than that of control group(P<0.05).There were no significant differences in cerebral blood flow velocity between the two groups(P>0.05);after treatment,the blood flow velocity of left and right vertebral artery and basilar artery in experimental group were higher than those in control group(P<0.05).There were no significant differences in hemorheology indexes between the two groups before treatment(P>0.05);after treatment,plasma viscosity,hematocrit and whole blood high tangential viscosity of experimental group were lower than those of control group(P<0.05).No serious adverse reactions occurred in both groups.Conclusion:Huatan Dingxuan Formula combined with Betahistine Hydrochloride Tablets has a positive effect in the treatment of senile vertigo,which can significantly improve the symptoms integrals,cerebral blood flow velocity
