黄斑格栅样激光光凝联合玻璃体注射雷珠单抗、曲安奈德治疗视网膜静脉阻塞性黄斑水肿的效果及对视网膜血流参数的影响

摘　　要：目的探究黄斑格栅样激光光凝联合玻璃体注射雷珠单抗、曲安奈德治疗视网膜静脉阻塞性黄斑水肿的效果。方法选择2019年1月至2022年10月我院收治的300例视网膜静脉阻塞性黄斑水肿患者为研究对象,以随机数字表法将其分为对照组和观察组,各150例。对照组接受黄斑格栅样激光光凝+曲安奈德治疗,观察组在对照组基础上加施玻璃体注射雷珠单抗治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗3个月后,观察组的视网膜中央收缩期峰值流速(PSV)、舒张期峰值流速(EDV)高于对照组(P<0.05)。出院时,观察组的结缔组织生长因子(CTGF)、血小板源性生长因子-C(PDGF-C)及血管内皮生长因子(VEGF)水平低于对照组(P<0.05)。治疗3个月后,观察组的三叶草像差、总高阶像差、球差及彗差优于对照组(P<0.05)。结论黄斑格栅样激光光凝联合玻璃体注射雷珠单抗、曲安奈德治疗视网膜静脉阻塞性黄斑水肿效果满意,可调节生长因子表达,改善视网膜血流参数及视觉质量,值得推广。Objective To explore the effect of macular grid laser photocoagulation combined with intravitreal injection of ranibizumab and triamcinolone acetonide in the treatment of retinal vein occlusion macular edema.Methods A total of 300 patients with retinal vein occlusion macular edema admitted in our hospital from January 2019 to October 2022 were selected as the research objects,and the patients were divided into control group and observation group by random number table method,with 150 cases in each group.The control group was treated with macular grid laser photocoagulation+triamcinolone acetonide,and the observation group was treated with intravitreal injection of ranibizumab on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After 3 months of treatment,the peak systolic velocity(PSV)and end diastolic velocity(EDV)of the central retina in the observation group were higher than those in the control group(P<0.05).At discharge,the levels of connective tissue growth factor(CTGF),platelet derived growth factor-C(PDGF-C)and vascular endothelial growth factor(VEGF)in the observation group were lower than those in the control group(P<0.05).After 3 months of treatment,the trefoil aberration,total higher-order aberration,spherical aberration and coma aberration of the observation group were better than those of the control group(P<0.05).Conclusion Macular grid laser photocoagulation combined with intravitreal injection of ranibizumab and triamcinolone acetonide is effective in the treatment of retinal vein occlusion macular edema,it can regulate the expression of growth factors,improve retinal blood flow parameters and visual quality,which is worthy of promotion.
