摘　　要：目的研究ERCC1基因多态性对肺癌合并胸腔积液易感性的影响。方法各选取90例肺癌合并恶性胸腔积液患者、肺癌无胸腔积液患者和健康体检者,分别对三组人群的基因型、最小等位基因频率进行对比观察。结果三组吸烟情况和家庭肿瘤史情况比较具有统计学差异(P<0.05)。三组的基因表达之间的差异存在统计学意义(P<0.05)。肺癌合并胸腔积液组的ERCC1基因表达水平均高于肺癌组与健康组。通过对三组ERCC1基因型进行比较,发现ERCC1基因类型中的C19007T以及C8092A具有统计学差异(P<0.05)。生存分析表明高表达ERCC1的肺癌患者具有更短的生存期(P=0.011,P=0.047)。结论ERCC1基因类型中的C19007T和C8092A与肺癌合并恶性胸腔积液患者易感性呈现正相关,在疾病的进展与早期发现中具有积极意义。Objective To investigate the effect of ERCC1 gene polymorphism on susceptibility to lung cancer complicated with pleural effusion.Methods 90 patients with lung cancer complicated with malignant pleural effusion,90 lung cancer patients without pleural effusion and 90 healthy subjects were selected,and the genotypes and minimum allele frequencies of the three groups were compared and observed.Conclusion There were significant differences in smoking status and family tumor history among the 3 groups(P<0.05).The expression level of ERCC1 gene in lung cancer patients with malignant pleural effusion was higher than that in lung cancer patients without pleural effusion and healthy patients.There was significant difference in gene expression between the disease group and the control group(P<0.05).By comparing the ERCC1 genotypes among the three groups,it was found that C19007T and C8092A of ERCC1 genotypes were statistically different(P<0.05).Survival analysis showed that lung cancer patients with high ERCC1 expression had shorter survival(P=0.011,P=0.047).Conclusion C19007T and C8092A of ERCC1 gene types are positively correlated with the susceptibility of patients with lung cancer complicated with malignant pleural effusion,which has positive significance in the progression and early detection of the disease.
