影响阻塞性睡眠呼吸暂停低通气综合征患者病情的解剖学因素分析

摘　　要：目的对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的多导睡眠监测(PSG)和上气道CT进行分析,探寻影响OSAHS严重程度的解剖学因素。方法回顾性分析2022年1月至2023年11月就诊于中国科学技术大学附属第一医院(安徽省立医院)的78例OSAHS患者病历资料,对OSAHS患者的PSG及上气道CT进行分析。结果体重指数与软腭直径具有正相关性(r=0.359,P<0.05);呼吸暂停低通气指数与软腭直径具有相关性(r=0.258,P<0.05);平均血氧饱和度与软腭长度具有相关性(r=-0.330,P<0.05);最低血氧饱和度与会厌游离缘水平气道面积具有相关性(r=0.266,P<0.05);动脉血氧饱和度小于90%的时间占总睡眠时间的比例与软腭长度具有相关性(r=0.271,P<0.05);动脉血氧饱和度小于90%的时间占总睡眠时间的比例与软腭直径具有相关性(r=0.269,P<0.05)。结论OSAHS严重程度主要受软腭的直径和长度影响,对软腭的处理是手术治疗的关键。Objective By analyzing PSG(Polysomnography)and upper airway CT to obtain anatomical factors that affect the severity of Obstructive sleep apnea/hypopnea syndrome OSAHS(Obstructive Sleep Apnea-Hypopnea Syndrome).Methods A retrospective analysis was conducted in 78 patients with obstructive sleep apnea syndrome(OSAHS)who visited the First Affiliated Hospital of University of Science and Technology of China from January 2022 to November 2023.The correlation between PSG and upper airway CT of OSAHS patients was analyzed.Results The following points were significantly correlated:the body mass index(BMI)and soft palate diameter(r=0.359,P<0.05);the apnea/hypopnea index(AHI)and soft palate diameter(r=0.258,P<0.05);the average blood oxygen saturation(MSaO 2%)and soft palate length(r=-0.330,P<0.05);the lowest blood oxygen saturation(LSaO 2%)and the airway area at the level of the free edge of the epiglottis(r=0.266,P<0.05);the cumulative time with oxygen saturation less than 90%(CT 90)and soft palate length(r=0.271,P<0.05);CT 90 and soft palate diameter(r=0.269,P<0.05).Conclusions The severity of OSAHS is mainly affected by the diameter and length of the soft palate,and the treatment of the soft palate is the key to surgical treatment.
