摘 要:借助福柯的现代权力理论,尝试将微观意义上的慢性病患者的疾痛叙事与宏观的权力结构联系起来进行分析,试图发现个体疾痛叙事所展现的现代权力关系及其背后更加宏观的结构性力量。慢性病疾痛体验所表达的疾痛叙事背后隐藏着宏观的现代权力结构,医疗场域中的文化符号、资本与市场、现代医学、医疗制度等形成了一个无所不在的权力网络,不断生产着医疗卫生领域的主流话语,并在具体的临床实践和日常生活世界中通过种种权力技术加以运作,形塑和支配着慢性病患者的疾痛叙事。对医疗场域中的权力结构进行反思,或许可以为缓解慢性病患者及其家庭的疾痛提供一种更加宏观的思路,也从疾痛叙事的角度为医疗领域的相关改革增添人性化的色彩。This article uses Foucault's modern power theory to analyze relationship between the micro-level illness narrative of chronic disease patients and the macro-level power structure,in an attempt to uncover the modern power relations and underlying structural forces reflected in individual illness narratives.The illness narrative of chronic disease expressed through patients'experience hides a macro modern power structure.Cultural symbols,capital and markets forces,modern medicine,healthcare systems,and other factors in the medical field form a ubiquitous power network,constantly producing mainstream discourse in the field of healthcare.Through various power technologies,it operates in specific clinical practices and daily life worlds,shaping and dominating the illness narrative of patients with chronic disease.Reflection on the power structure in the medical field may provide a more comprehensive perspective for alleviating the suffering of chronic disease patients and their families,and also add a humanistic element to relevant reforms in the medical field from the perspective of illness narrative.
