巩膜瓣可调节缝线联合小梁切除术对闭角型青光眼患者术后眼压及浅前房发生的影响

摘　　要：目的探讨闭角型青光眼(PACG)患者采用巩膜瓣可调节缝线联合小梁切除术(TRA)的临床效果。方法按随机数字表法将2022年10月至2023年8月于我院治疗的84例(96眼)PACG患者分为2组,各42例(48眼)。对照组采用TRA治疗,观察组采用巩膜瓣可调节缝线联合TRA治疗。比较2组临床疗效、眼压、最佳矫正视力(BCVA)、视功能相关生活质量、实验室指标及并发症。结果观察组治疗总有效率高于对照组,治疗后眼压、BCVA低于对照组,视力相关的生活质量量表(LVQOL)评分高于对照组,治疗后碱性成纤维细胞生长因子(bFGF)及CXC趋化因子受体2(CXCR2)水平低于对照组,浅前房、非功能性滤过泡、巩膜瓣粘连总发生率低于对照组,差异有统计学意义(P<0.05)。结论PACG患者采用巩膜瓣可调节缝线与TRA联合治疗眼压控制效果更好,可降低bFGF及CXCR2水平,改善视力,减少并发症,提高患者视功能相关生活质量。Objective To explore the clinical efficacy of adjustable scleral flap suture combined with trabeculectomy(TRA)in patients with angle closure glaucoma(PACG).Methods A total of 84 PACG patients(96 eyes)treated in our hospital from October 2022 to August 2023 were divided into two groups using a random number table method,with 42 cases(48 eyes)in each group.The control group received TRA treatment,while the observation group received adjustable scleral flap suture combined with TRA treatment.The clinical efficacy,intraocular pressure,best corrected visual acuity(BCVA),visual function-related quality of life,and complications between the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group,and after treatment,the intraocular pressure and BCVA were lower than those of the control group.The LVQOL score related to low vision was higher than that of the control group,and the total incidence of complications was lower than that of the control group,with statistical differences(P<0.05).Conclusion The combined treatment of adjustable scleral flap suture and TRA in PACG patients has a better effect on controlling intraocular pressure,improving vision,reducing complications,and enhancing the quality of life related to visual function.
