癫痫样放电对颞叶癫痫患者睡眠时长的影响

摘　　要：目的探讨癫痫样放电对颞叶癫痫(TLE)患者睡眠时长的影响。方法选择2022年1月至2023年12月于医院行视频脑电图(VEEG)监测的40例TLE患者,根据患者夜间睡眠中是否发生癫痫样放电分为放电组(20例)和无放电组(20例)。比较两组总睡眠时间(TST)、非快速眼动期睡眠(NREM)总时长、NREM各分期N1、N2、N3时长及占所睡眠时间比例、快速眼动期(REM)潜伏期时长、REM时长及占TST比例等。结果放电组TST短于无放电组,差异有统计学意义(P<0.05);放电组REM总时长短于无放电组,差异有统计学意义(P<0.05);放电组REM占TST比例小于无放电组,差异有统计学意义(P<0.05);放电组NREM期比例、N1期比例高于无放电组,差异有统计学意义(P<0.05)。结论癫痫样放电可能会影响TLE患者的睡眠时长,临床中应密切关注其睡眠状况,及时识别和处理癫痫样放电,以改善患者睡眠质量。Objective To investigate the impact of epileptiform discharge on sleep duration and structure in patients with temporal lobe epilepsy through video EEG(VEEG)monitoring.Methods A total of 40 patients with temporal lobe epilepsy who were hospitalized in Hospital for video EEG monitoring were selected and divided into two groups according to whether or not they had epileptiform discharge during night sleep.The total sleep duration(TST),duration and proportion of each sleep stage were compared between the two groups.Results The TST of the group with discharge was shorter than that of the group without discharge,and the difference was statistically significant(P<0.05).The total duration of REM in the discharge group was significantly shorter than that in the non discharge group(P<0.05).The proportion of REM to TST in the discharge group was lower than that in the non discharge group,and the difference was statistically significant(P<0.05).The proportion of NREM stage and N1 stage in the discharge group was higher than that in the non discharge group,and the difference was statistically significant(P<0.05).Conclusion Epilepsy like discharges may affect the sleep duration of patients with TLE.In clinical practice,close attention should be paid to the sleep status of TLE patients,and epileptic like discharges should be identified and treated in a timely manner to improve their sleep quality.
