血清降钙素、血管内皮生长因子与异常糖链糖蛋白联合检测对甲状腺癌的诊断价值

摘　　要：目的探讨血清降钙素(CT)、血管内皮生长因子(VEGF)与异常糖链糖蛋白(TAP)联合检测对甲状腺癌的诊断价值。方法回顾性分析我院2022年3月至2023年12月收治的64例甲状腺癌患者的临床资料作观察组,另选择同期66例甲状腺良性结节患者作为对照组,分析、整理所有研究对象的基本资料,均接受血清CT、VEGF、TAP检验。对比两组血清CT、VEGF、TAP水平;分析引发甲状腺癌的影响因素;分析血清CT、VEGF、TAP单独及联合检测甲状腺癌的诊断价值。结果观察组血清CT、VEGF、TAP水平均高于对照组,差异有统计学意义(P<0.05);血清CT(OR=1.950,95%CI 1.765~2.155)、VEGF(OR=2.059,95%CI 1.791~2.366)、TAP(OR=2.206,95%CI 1.930~2.520)均是引发甲状腺癌的独立危险因素(P<0.05);血清CT、VEGF、TAP水平联合检测诊断甲状腺癌曲线下面积(AUC)值高于单项检测(Z=3.027、3.525、2.846,P<0.05)。结论血清CT、VEGF、TAP联合检测诊断甲状腺癌的价值较高,可作为辅助诊断该疾病的主要参考依据。Objectives To investigate the diagnostic value of combined detection of serum calcitonin(CT),vascular endothelial growth factor(VEGF)and tumor abnormal protein(TAP)in the diagnosis of thyroid cancer.Methods The clinical data of 64 patients with thyroid cancer admitted to my Hospital from March 2022 to December 2023 were retrospectively selected as the observation group,and 66 patients with benign thyroid nodule during the same period were selected as the control group.The basic data of all subjects were analyzed and organized,and serum CT,VEGF and TAP tests were performed.Serum CT,VEGF and TAP levels were compared between the two groups.The influencing factors of thyroid carcinoma were analyzed.The diagnostic value of serum CT,VEGF and TAP alone and combined in detecting thyroid cancer were analyzed.Results The serum levels of CT,VEGF and TAP in the observation group were significantly higher than those in control group(P<0.05).Serum CT(OR=1.950,95%CI 1.765~2.155),VEGF(OR=2.059,95%CI 1.791~2.366)and TAP(OR=2.206,95%CI 1.930~2.520)were independent risk factors for thyroid cancer(P<0.05).The value of area under the curve(AUC)of combined detection of serum CT,VEGF and TAPwas significantly higher than that of single detection(Z=3.027,3.525,2.846,P<0.05).Conclusions The combined detection of serum CT,VEGF and TAP is of high value in the diagnosis of thyroid cancer,which can be used as the main reference for the auxiliary diagnosis of the disease.
