彩色多普勒超声结合超声造影参数在卵巢癌诊断中的应用价值

摘　　要：目的探究彩色多普勒超声(CDUS)结合超声造影(CEUS)参数在卵巢癌诊断中的应用价值。方法回顾性选取收治的卵巢占位性病变患者73例,根据病理结果分为良性组(n=40)和恶性组(n=33),根据是否发生淋巴结转移将恶性组分为转移组(n=16)和非转移组(n=17),收集并比较各组一般资料、CDUS及CEUS结果差异。评价CDUS及CEUS结果对卵巢良恶性肿瘤及其淋巴结转移情况的预测价值。结果恶性组阻力指数(RI)、搏动指数(PI)、到达时间(AT)及达峰时间(TTP)低于良性组,峰值流速(PSV)高于良性组(P<0.05);转移组RI、PI、AT低于非转移组,PSV、TTP高于非转移组(P<0.05);RI、PI、AT、TTP及PSV均对卵巢良恶性肿瘤及其淋巴结转移情况有一定的鉴别诊断价值,其中五者联合检测鉴别诊断效能均最高。结论CDUS联合CEUS参数有利于提高卵巢良恶性肿瘤及其是否发生淋巴结转移的诊断效能。Objective To explore the application value of color Doppler ultrasound(CDUS)combined with con-trast-enhanced ultrasound(CEUS)for diagnosing ovarian cancer.Methods 73 patients with ovarian space-occupying lesions were retrospectively selected.The patients were divided into the benign group(n=40)and the malignant group(n=33)according to the pathological examination results,and the malignant group was sub-divided into the metastasis group(n=16)and the non-metastasis group(n=17)by whether the patients had lymph node metastasis.The general data,CDUS and CEUS differences were collected and compared.The predictive value of CDUS and CEUS results for ovarian tumors(benign or malignant)and lymph node metastasis was evaluated.Results The resistance index(RI),pulse index(PI),arrival time(AT)and time to peak(TTP)in the malignant group were lower than those in the benign group,and the peak systolic velocity(PSV)was higher(P<0.05);RI,PI and AT in the metas-tasis group were lower than those in the non-metastasis group,PSV and TTP were higher than those in the non-me-tastasis group(P<0.05);RI,PI,AT,TTP and PSV all had a certain value for differential diagnosis of ovarian tumors(benign or malignant)and their lymph node metastasis,and the combined detection of the five parameters had the highest differential diagnosis efficiency.Conclusions CDUS combined with CEUS parameters can improve the di-agnostic efficiency of ovarian tumors and the incidence of lymph node metastasis.
