摘 要:目的观察乳腺癌患者人乳头状瘤病毒(HPV)感染的临床病理特征及与p53蛋白表达的相关性。方法选取180例乳腺癌患者,根据其是否感染HPV分为感染组(n=91)和未感染组(n=89),比较2组患者癌组织和癌旁组织的p53蛋白表达水平、临床病理特征、生存情况,以及2组不同临床病理特征的p53蛋白表达水平。结果感染组组织学分级、病理分期与未感染组相比,差异均有统计学意义(P均<0.05);感染组淋巴结转移占65.93%,高于未感染组的43.82%(χ^(2)=8.890,P<0.01)。感染组累积生存率、无进展生存率分别为82.26%、66.13%,低于未感染组的95.16%、90.32%,差异均有统计学意义(log-rank χ^(2)=5.766、13.220,P均<0.001)。感染组患者癌组织及癌旁组织的p53蛋白表达水平分别为0.58(0.45~0.60)、0.24(0.21~0.28),未感染组分别为0.42(0.35~0.46)、0.21(0.18~0.25),差异均有统计学意义(Z=8.525、2.923,P均<0.01)。感染组与未感染组在肿瘤直径、组织学分级、病理分期、有无淋巴结转移的p53蛋白表达水平比较,差异均有统计学意义(P均<0.05)。结论乳腺癌患者HPV感染率较高,HPV感染可能促使p53蛋白突变,促进乳腺癌恶性发展。Objective This study was aimed at observing the clinicopathological characteristics of human papilloma virus(HPV)infection and their correlations with p53 protein expression in patients with breast cancer.Methods A total of 180 patients with breast cancer were enrolled and divided into an infection group(n=91)and non-infection group(n=89),according to the presence or absence of HPV infection.The expression of p53 protein in cancer tissues and para-carcinoma tissues,clinicopathological data,survival,and clinicopathological characteristics were compared between groups.Results Significant differences in histological grading and pathological staging were observed between groups(both P<0.05);lymph node metastasis was observed in 65.93%of the infection group and 43.82%non-infection group(χ^(2)=8.890,P<0.01).The cumulative survival rate and progression-free survival rate in the infection group were 82.26%and 66.13%,respectively,and were significantly lower than those in the non-infection group(95.16%,90.32%)(log-rankχ^(2)=5.766,13.220,both P<0.001).The expression levels of p53 protein in cancer tissues and para-carcinoma tissues in the infection group were 0.58(0.45-0.60)and 0.24(0.21-0.28),whereas those in the non-infection group were 0.42(0.35-0.46)and 0.21(0.18-0.25);the differences between groups were statistically significant(Z=8.525,2.923,both P<0.01).Significant differences in p53 protein expression were observed among patients according to tumor diameter,histological grading,pathological staging,and lymph node metastasis between the infection group and non-infection group(all P<0.05).Conclusion The incidence of HPV infection was elevated in patients with breast cancer,HPV infection may promote the mutation of p53 protein and the malignant development of breast cancer.
