摘　　要：目的:评价C臂透视引导下经椎弓根入路行骶骨成形术治疗骶1椎体病变的可行性、近期疗效及安全性。方法:回顾性分析2018年6月~2021年6月无锡市人民医院17例C臂透视引导下行经皮骶骨成形术治疗骶1椎体病变的患者资料,定期评价术后1年内的临床疗效,并记录术后并发症情况。结果:17例骶1椎体成形术均获成功,穿刺点棘突旁开平均距离(4.5±0.5)cm,骨水泥(3.8±1.1)mL/节。视觉模拟评分(VAS):术前(7.47±1.33)分,术后1d(3.06±1.15)分,术后7d(2.69±0.48)分,术后1个月(2.63±0.62)分,术后6个月(2.81±0.54)分,术后1年(2.79±0.70)分。术后CT证实1例骶管渗漏,2例椎间盘渗漏,均无临床症状,无穿刺路径损伤相关并发症。结论:C臂透视引导下经椎弓根入路行骶骨成形术治疗骶1椎体病变技术安全可行,近期止痛效果显著。Objective:To evaluate the feasibility,short-term efficacy and safety of C-arm fluoroscopic guided transpedicle approach sacrplasty for the treatment of sacral 1 vertebral lesions.Methods:From June 2018 to June 2021,the data of 17 patients in Wuxi People’s Hospital who underwent percutaneous sacral arthroplasty under C-arm fluoroscopy guidance for the treatment of sacral 1 vertebral body lesions were retrospectively analyzed.The clinical efficacy within 1 year after surgery was regularly evaluated,and the postoperative complications were recorded.Results:All the 17 cases of sacral 1 vertebroplasty were successful.The average paracanthoplasty distance was(4.5±0.5)cm and the bone cement was(3.8±1.1)mL/segment.Visual analogue score(VAS):(7.47±1.33)points before surgery,(3.06±1.15)points 1 day after surgery,(2.69±0.48)points 7 days after surgery,(2.63±0.62)points 1 month after surgery,(2.81±0.54)points 6 months after surgery,and(2.79±0.70)points 1 year after surgery.Postoperative CT confirmed 1 case of sacral canal leakage and 2 cases of intervertebral disc leakage,with no clinical symptoms and no complications related to puncture path injury.Conclusion:The C-arm fluoroscopic guided transpedicle approach sacral arthroplasty is a safe and feasible technique for the treatment of sacral 1 vertebral body lesions,and the short-term analgesic effect is significant.
