摘 要:目的采用Milliplex多因子检测技术检测人恒牙炎症牙髓组织中多种炎性细胞因子的表达,探讨炎症因子变化能否作为辅助评估牙髓炎症程度的指标。方法选取2019年3月至2021年3月,128名就诊于首都医科大学附属北京口腔医院的患者,按临床牙髓炎症程度由轻到重分为四组:正常牙髓组(正常组,n=30)、轻度牙髓炎组(轻度组,n=32)、中度牙髓炎组(中度组,n=31)、重度牙髓炎组(重度组,n=35)。收集患牙穿髓孔处的牙髓组织渗出液,用Milliplex多因子检测技术检测样本中GM-CSF、IFN-γ、IL-10、IL-17A、IL-1β、IL-6、IL 8以及TNF-α的浓度并进行统计分析。结果随着牙髓炎症程度的加重,牙髓组织渗出液中促炎因子IL-6、IL-8浓度各组间浓度差异明显,呈明显递增趋势,组间两两比较均有显著性差异;IL-1β浓度总体呈递增趋势,重度组明显增高,正常组、轻度组、中度组与重度组间、正常组与中度组间有统计学差异;TNF-α、IL-17A浓度在重度组有所增高,较正常组、轻度组和中度组有显著性差异,但各组间数值差异较小;IFN-γ、GM-CSF、IL-10各组间浓度变化不明显。结论随着牙髓炎症的进展,牙髓组织渗出液中IL-6、IL-8浓度呈明显递增趋势,有可能作为评估牙髓炎症状态的指标,IL-8灵敏度最高,可能成为关键指标。IL-1β浓度在重度牙髓炎中明显升高,可能与牙髓炎症的急性发展有关。Objective To detect the expression of various inflammatory cytokines in the pulp tissue of permanent teeth under different inflammatory states,and explore whether changes in inflammatory factors can serve as an auxiliary indicator for evaluating the degree of pulp inflammation.Methods From March 2019 to March 2021,128 patients who visited Beijing Stomatological Hospital were divided into four groups according to the degree of clinical pulpitis:normal pulp group(normal group,n=30),mild pulpitis group(mild group,n=32),moderate pulpitis group(moderate group,n=31),and severe pulpitis group(severe group,n=35).Collect the exudate of dental pulp tissue at the perforation of the affected tooth,and use Milliplex multi factor detection technology to detect the concentration of GM-CSF,IFN-γ,IL-10,IL-17A,IL-1β,IL-6,IL-8,TNF-α.Results As the pulp inflammation worsened,the concentrations of pro-inflammatory factors IL-6 and IL-8 in the exudate of pulp tissue showed significant differences among groups,with a significant increasing trend.There were significant differences between the two groups compared.Concentration of IL-1βshowed an overall increasing trend,with a significant increase in the severe group.There were statistical differences between the normal group,mild group,moderate group and severe group,as well as between the normal group and moderate group.The concentration of TNF-αand IL-17A increased in the severe group,with a significant difference compared to the normal group,mild group,and moderate group,but the numerical differences between each group was relatively small.The concentration changes between IFN-γ、GM-CSF and IL-10 groups were not significant.Conclusions With the progression of dental pulp inflammation,the concentrations of IL-6 and IL-8 in the exudate of dental pulp tissue show a significant increasing trend,which may serve as an indicator for evaluating the status of dental pulp inflammation.IL-8 has the highest sensitivity and may be a key indicator.The concentration of IL-1βsignificantly inc
